Peri-implantitis is a progressive inflammatory condition affecting the tissues around dental implants, posing significant challenges to implant success. This study explores the etiology, diagnostic techniques, and treatment modalities for peri-implantitis, focusing on evidence-based approaches and innovative therapies to improve clinical outcomes and ensure long-term implant survival. Methodology: A comprehensive review of existing literature was conducted, including studies on non-surgical, surgical, and adjunctive treatment methods. The research included clinical trials, meta-analyses, and systematic reviews to assess treatment outcomes. Key diagnostic techniques, such as clinical probing, radiography, cone-beam computed tomography (CBCT), and microbial testing, were analyzed for their efficacy in disease classification and early detection. Aim: The aim of this study is to investigate the etiology, diagnostic techniques, and treatment modalities for peri-implantitis. The objectives include evaluating evidence-based practices, exploring innovative therapies for biofilm management, and identifying strategies to enhance tissue regeneration and long-term implant success. Results: The findings indicate that treatment strategies depend on the stage of peri-implantitis. Non-surgical approaches, such as mechanical debridement and adjunctive therapies (e.g., antimicrobial photodynamic therapy and local antibiotics), are effective in the early stages of the disease. In advanced cases, surgical interventions, including regenerative techniques with bone grafts, barrier membranes, and implantoplasty, demonstrated significant improvements in tissue health and microbial load reduction. Emerging therapies, such as electrolytic cleaning, ozone therapy, and stem cell-based approaches, show promising potential for enhancing treatment outcomes. Conclusion: A multi-modal approach combining non-surgical and surgical interventions, tailored treatment plans, and regular maintenance care is essential for successful peri-implantitis management. Future advancements, particularly in bioactive materials and regenerative therapies, hold the potential to improve clinical outcomes and ensure long-term implant success.
