Ievads: Bērnu neatliekamās medicīniskās palīdzības gadījumos intraosālās (IO) pieejas lietošana ir pazīstama metode un tiek stingri atbalstīta kā ātra alternatīva intravenozai pieejai vairākās noteikts klīniskajās vadlīnijās. Piekļuves vienkāršība, mazais laika patēriņš, līdzvērtīga efektivitāte kā intravenozai ievadīšanai un fakts, ka IO kalpo kā venoza pieeja, kas nesaplok – tas viss noved pie tā, ka IO ceļš tiek aizvien biežāk izmantots zāļu un šķidruma ievadīšanai neatliekamās situācijās pediatrijā. Tāpat kā jebkura invazīva procedūra, IO piekļuve un infūzija rada dažādus potenciālus riskus un komplikācijas, kas jāpatur prātā un, kad vien iespējams, ir jānovērš. Mērķis: Šī pētījuma mērķis ir apkopot, pārskatīt un izcelt pašreizējo medicīnisko literatūru, kurā aprakstītas stilba kaula IO piekļuves komplikācijas pediatriskajiem pacientiem, lai identificētu visbiežāk sastopamās, smagākās komplikācijas un, visbeidzot, identificētu pasākumus potenciālo komplikāciju novēršanai. Metodes: Literatūras pārskats par rakstiem angļu valodā, kuros ziņots par stilba kaula IO komplikācijām vecumā no jaundzimušajiem līdz 18 gadu vecumam, izmantojot tiešsaistes datubāzes PubMed un Google Scholar. Autors sarežģījumiem piešķīra “riska pakāpi”, izmantojot subjektīvu 5x5 riska matricas tabulu sistēmu “HASpod- Better Health and Safety Management”. Rezultāti: Šajā pārskatā bija iekļauti 39 raksti — 18 pētījumi un 22 gadījumu ziņojumi. Tika identificētas 27 komplikācijas, biežākās komplikācijas ir ekstravazācija, stilba kaula lūzums un compartment sindroms. Smagākās komplikācijas ir akūta ekstremitāšu išēmija un audu nekroze. Veiksmīga IO adatas ievietošanas varbūtība ar pirmo reizi, ja tiek ziņota, ir zema. Aprakstītie komplikāciju veidi un biežums literatūrā ir ļoti atšķirīgs. Secinājumi: Ekstravazācija, lai arī ir visizplatītākā komplikācija, darbojas arī kā sāpju, compartment sindroma un išēmijas katalizators. Pareiza IO pieejas vietas lokalizācija, adekvāta tehnika, efektīva fiksācija, nepārtraukta punkcijas vietas un ekstremitāšu novērošana un agrīna IO adatu izņemšana var būt noderīgi kā preventīvi pasākumi stilba kaula IO komplikācijām bērniem.
Background: In pediatric medical emergencies, the use of Intraosseous (IO) access is well established and strongly advocated as a rapid alternative to Intravenous access as stated by many clinical guidelines. The ease of access, the short time required, the equivalent efficacy to the intravenous route, and the fact that the IO serves as a non-collapsible venous site- all result in the increased use of the IO route for drug and fluid administration, during pediatric emergencies. As with any invasive procedure, the IO access and infusion pose some potential risks and complications which should be kept in mind and should be prevented, whenever possible. Aim: The aim of this research is to compile, review, and highlight the current medical literature which describes complications of tibial IO access in pediatric patients, in aim to identify the most common complications, the most severe complications, and lastly, to present potentially preventive measures for the complications. Methods: Literature review of English articles reporting tibial IO complications in children from birth up to age of 18-years, using the online database, PubMed, and Google scholar. Additionally, the complications were designated a “Degree of risk” by the author, using the subjective 5x5 Risk-matrix table system by “HASpod- Better Health and Safety management”. Results: This review included 39 articles- 18 studies and 22 case reports. 27 complications were identified, the most common complications are extravasation, tibial fracture, and compartment syndrome. The most severe complications are acute limb ischemia and tissue necrosis. First-attempt success rates of IO needle insertion, when reported, were poor. The described complication types and rates vary widely in the literature. Conclusion: Extravasation, while being the most common complication, also acts as a catalyst for pain, compartment syndrome, and ischemia. Correct location, adequate technique, effective fixation, continuous observation of the puncture site and limb, and early removal of IO needles can be beneficial as preventive measures for tibial IO complications in pediatric patients.
