Pamatojums: Cerebrālās triekas izpausmes ir novērojamas jau no agras bērnības. Galvenokārt personām ar cerebrālo trieku ir ietekmētas un traucētas kustības un samazinātas posturālās spējas un līdzsvars. Samazinātas motorās spējas var novest pie personas nespējas mainīt vai noturēt ķermeņa balansu un pozīciju telpā, tādējādi palielinot ķermeņa deformāciju un asimetriju attīstību risku. Aptuveni 63% no personām ar cerebrālo trieku savu ikdienu pavada sēdus, tādējādi šīs personas izvēlas izmantot dažādus tehniskos palīglīdzekļus, lai atvieglotu vai uzlabotu sēdus pozīciju. Lielākoties šī sēdus poza ir deformācijas veicinoša vai pastiprinoša.
Pētījuma klīniskais jautājums (PIOS): Vai mainās posturālā stabilitāte un deformācijas sēdus pozīcijā, palielinoties personas Gross Motor Function Classification System līmenim?
Pētījuma mērķis: Veikt sistematizētu literatūras izpēti, lai apkopotu un izanalizētu pieejamo informāciju par posturālo stabilitāti un ar to saistītajām deformācijām cilvēkiem ar cerebrālo trieku, atrodoties sēdus.
Pētījuma metodes: Datu meklēšanai izmantotas piecas elektroniski bibliogrāfiskās datu bāzes: PubMed, Science Direct, SCOPUS, EBSCOhost un ProQuest. Iegūto datu atlase veikta, izmantojot PRISMA (Preffered Reporting Items for Systematic Review and Meta-Analyses) diagrammu. Pētījumu kvalitāte novērtēta pēc LEGEND (Let Evidence Guide Every New Decision) viena pētījuma kvalitātes novērtēšanas sistēmas, kas atlasīta šķērsgriezuma pētījumiem, kas tālāk atlasīta pēc veida “terapija (tostarp ārstēšana, terapija, profilakse, kaitējums un kvalitātes uzlabošana).
Rezultāti: Kvantitatīvā analīzē iekļauti trīs pētījumi, kuros starp atšķirīgām apskatītajām īpašībām, kas saistītas ar ķermeņa posturālajām spējām un stājas kvalitāti, visi rezultāti vienoti norāda uz ķermeņa posturālās asimetrijas un deformācijas sastopamību un tās palielināšanos, pieaugot Gross Motor Function Classification System līmenim.
Secinājumi: 1. Pētījuma mērķis ir sasniegts, apkopojot un izanalizējot pieejamo informāciju par posturālo stabilitāti un ar to saistītajām deformācijām cilvēkiem ar cerebrālo trieku, atrodoties sēdus. 2. Personām ar cerebrālo trieku, kas ikdienā pavada laiku sēdus, palielinoties Gross Motor Function Classification System līmenim, attiecīgi pazeminās funkcionēšanas līmenis, ietekmējot personas spējas, rezultātā tiek novērotas lielākas ķermeņa posturālās deformācijas un asimetrija.
Background: Cerebral palsy symptoms are observed from early childhood. Mainly, people with cerebral palsy have affected and disturbed movement and reduced postural abilities and balance. Reduced motor abilities lead to the person being unable to change or maintain the body’s balance and position, which can increase the risk of developing deformities and asymmetries of the body. Approximately 63% of people with cerebral palsy spend their daily lives sitting, so they or their caregivers choose various technical/medical aids to facilitate the sitting position. Most of the time, this prolonged sitting posture promotes or exacerbates the deformity. Clinical research question (PIOS): Does postural stability and deformations in the sitting position change as a person’s Gross Motor Function Classification System level increases? Objective: To conduct a systematic literature review to summarize and analyse available information on postural stability and related deformities in people with cerebral palsy while sitting. Methods: Five electronic bibliographic databases were used for data search: PubMed, ScienceDirect, SCOPUS, EBSCOhost and ProQuest. The selection of the obtained data was carried out using the PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses) diagram. The quality of the studies was assessed according to the LEGEND (Let Evidence Guide Every New Decision) single study quality assessment system, selected for cross-sectional studies, further selected by type of “Intervention (including Treatment, Therapy, Prevention, Harm, and Quality Improvement)”. Results: The quantitative analysis included three studies in which, among the different characteristics examined, related to body postural abilities and posture quality, all results unanimously indicated an increase in the incidence and likelihood of body postural asymmetry and deformation with increasing Gross Motor Function Classification System level. Conclusion: 1. The aim of the study has been achieved by summarizing and analysing available information on postural stability and related deformities in people with cerebral palsy while sitting. 2. In individuals with cerebral palsy who are sedentary, as the Gross Motor Function Classification System level increases, the level of functioning decreases accordingly, affecting the person’s abilities, resulting in greater postural deformations and asymmetry of the body.
