Aktualitāte: Plaukstas un plaukstas pamatnes locītavas traumatiski bojājumi ir iemesls gandrīz 20% traumatiska rakstura uzņemšanas nodaļas vizīšu un šo bojājumu incidence pēdējo 25 gadu laikā ir ar tendenci pieaugt. Locītavas sarežģītā anatomija apgrūtina traumatisku bojājumu iniciālu radioloģisku diagnostiku. Agrīna un precīza traumatisku bojājumu diagnostika ir pamats pareizas terapijas uzsākšanā. Vēlu diagnosticētu traumatisku bojājumu terapijas izvēle ir ierobežota, pacientam risks neatgūt pilnvērtīgu locītavas funkciju un zaudēt dzīves kvalitāti. Radioloģisku izmeklējumu nozīmēšanai uzņemšanas nodaļā plaukstas pamatnes locītavas traumatisku bojājumu gadījumā nav izstrādātas un ieviestas vienotas klīnisko kritēriju vadlīnijas. Efektīvas radioloģiskās diagnostikas vadlīnijas plaukstas pamatnes locītavas traumatisku bojājumu gadījumā ne tikai veicinātu terapijas rezultātus, bet uzlabotu materiālo un cilvēkresursu sadali veselības aprūpes iestādēs un samazinātu gaidīšanas rindas ambulatoriem un stacionāra radioloģiskiem izmeklējumiem.
Mērķis: Apkopot un analizēt pētījumos un literatūras avotos sniegto informāciju par plaukstas pamatnes locītavas traumatisku bojājumu radioloģisko diagnostiku.
Metodes: zinātniskās literatūras atlase Dynamed, Cochrane Library, Science Direct, Clinical Key, Pubmed. Tika meklēti literatūras avoti, kuros pielietotas un salīdzinātas radioloģiskās diagnostikas metodes plaukstas pamatnes locītavas traumatisku bojājumu gadījumos, kuros aprakstīti biežākie traumatisko bojājumu veidi, to pazīmes un klasifikācijas un kuros aprakstīta radioloģisko izmeklējumu norise, kā arī indikācijas. Tika atlasīt avoti, kuri publicēti laika posmā no 2000. līdz 2022. gadam.
Secinājumi: Pašlaik nav izstrādātas efektīvas klīnisko kritēriju vadlīnijas plaukstas pamatnes locītavas traumatisku bojājumu radioloģiskai diagnostikai, rekomendācija saglabājas uzņemšanas nodaļā veikt iniciālu plaukstas pamatnes locītavas rentgenogrammu pie katra trula rakstura traumatiska bojājuma.
Topicality: Hand and wrist traumatic injuries are the cause for 20% of emergency department visits. The incidence of such trauma has been on a steady rise for the past 25 years. The complex anatomy of wrist joint makes initial diagnostics of such injury challenging. Early and precise traumatic injury diagnostics is key for choosing suited therapy methods. Initially misdiagnosed cases or cases of late diagnosis can lead to complications of the injury and leave a severe impact on patients’ further quality of life. Currently there are no effective clinical criteria guidelines for radiological imaging in case of acute wrist trauma. In case of such guidelines, not only appropriate treatment would be decided upon earlier, but they would also alleviate the workload for health care specialists. The aim of the research is to analyze and summarize the available scientific literature about wrist joint traumatic injury imaging. Methods: scientific literature was selected from Dynamed, Cochrane Library, Science Direct, Clinical Key, Pubmed. These databases were searched for studies that were about radiological imaging methods used to diagnose wrist joint trauma, most common types of wrist trauma, their signs and respectable classifications used, as well as the imaging process itself and their discussed indications. Studies were selected from year 2000 to 2022. Conclusion: Currently there are no effective clinical criteria guidelines for radiological imaging in case of acute wrist trauma, therefore the recommendation stays to perform an initial wrist radiograph for every person that visits the emergency department with a blunt wrist trauma.
