Šajā literatūras apskatā ir apkopoti pieejamie pētījumi un atklājumi par probiotiku lomu pacientu ārstēšanā, analizējot to darbības mehānismu, efektivitāti un drošību. Literatūra tika meklēta un identificēta, izmantojot plašu PubMed meklēšanu, tostarp gadījumu ziņojumus, metaanalīzes, klīniskos pētījumus, sistemātiskus pārskatus un pārskatus.
Probiotikas pēdējā laikā ir izraisījušas sabiedrības interesi kā daudzsološs veids, kā uzlabot cilvēka zarnu veselību un uzlabot vispārējo labsajūtu, un tiek pētītas, lai tās varētu kļūt par jaunu adjuvantu terapiju dažādām zarnu slimībām, piemēram, kairinātas zarnas sindromam, peptiskajai čūlai un aptaukošanās slimībām, pateicoties to spējai mainīt zarnu mikrofloru (Kim et al., 2019).
Indivīdiem ar iekaisīgu zarnu slimību un kairinātas zarnu sindromu probiotikas ir parādījušas uztura un imunoloģiski modulējošu iedarbību (Abraham et al., 2017). Citas priekšrocības ir redzamas ar antibiotikām saistītas caurejas ārstēšanā, kur probiotikas darbojas, kolonizējot zarnas, samazinot apkārtējās vides pH un izdalot eksotoksīnus (Goodman, et al., 2021). Turklāt pacienti ar vulvovaginālu kandidozi ziņoja par diskomforta uzlabošanos, kā arī samazinātu maksts eritēmu un izdalījumiem pēc 1 nedēļas ilgas probiotiku terapijas (Williams, 2010).
Kopumā probiotikas tiek uzskatītas par drošām, taču kā dzīvi organismi tie var ietekmēt mikrobiotu, imunitāti un citus fizioloģiskus procesus, kam nepieciešama lielāka uzmanība. Šie aspekti prasa plašākas diskusijas, īpaši pacientu grupās ar nomāktu imūnsistēmu (Merenstein et al., 2023).
Noslēgumā ir svarīgi pieminēt, ka klīniskās vadlīnijas iesaka probiotikas kā adjuvantu ārstēšanu ar antibiotikām saistītas caurejas, KZS un IZS gadījumos, tomēr, lai gan vairāki pētījumi liecina, ka probiotikām ir liels potenciāls, ir nepieciešami turpmāki pētījumi, lai noteiktu standartizētās vadlīnijas. visefektīvākie celmi, devas un ārstēšanas ilgums.
This literature review summarizes the available studies and findings about the role of probiotics in patients’ treatment by analyzing its mechanism of action, efficiency, and safety. Literature was searched and identified by an extensive PubMed search including case reports, meta-analyses, clinical trials, systematic reviews, and reviews. Probiotics have recently piqued the public's interest as a promising way of boosting human intestinal health and improving general wellbeing and are being examined in the prospective of becoming new adjuvant therapy for a variety of intestinal illnesses such as IBS, peptic ulcer, and obesity due to their ability to change the gut microflora (Kim et al., 2019). In individuals with inflammatory bowel disease and irritable bowel syndrome probiotics have shown nutritional and immunological modulatory effects (Abraham et al., 2017). Other benefits are seen in the treatment of antibiotic- associated diarrhea, where probiotics work by colonizing the gut, reducing ambient pH and emitting exotoxins (Goodman, et al.,2021). Furthermore, patients with vulvovaginal candidiasis reported an improvement in discomfort as well as reduced vaginal erythema and discharge after 1 week probiotic treatment (Williams,,2010). Overall probiotics are considered to be safe, but as living organisms, they can impact microbiota, immunity and other physiological processes, requiring more attention. These aspects require more discussion, especially in immunosuppressed groups of patients (Merenstein et al., 2023). In conclusion, it is important to mention that clinical guidelines recommend probiotics as adjuvant treatment in cases of antibiotic- associated diarrhoea, IBS and IBD, however while multiple studies show that probiotics hold big potential, further study is needed to determine the standardized guidelines, the most effective strains, doses, and treatment durations
