Pamatojums: Hronisks muguras lejasdaļas sāpes ir plaši izplatīta problēma visā pasaulē. Muguras lejasdaļas sāpes ir pierādītas par būtisku iemeslu funkcionālai nespējai cilvēkiem darbspējīgā vecumā, kas rada ekonomisko slogu kā indivīdam tā visai pasaulei. Ir pierādīts, ka muguras sāpes ir izturīgas pret pārmaiņām un joprojām trūkst skaidru pierādījumu, ka viena konkrēta ārstēšanas metode būtu labāka par citu muguras lejasdaļas sāpju gadījumos.
Mērķis: Apkopot un analizēt pieejamo literatūru vai kognitīvi funkcionālā terapija ir efektīvāka metode hronisku muguras lejasdaļas sāpju un nespējas mazināšanā par konvencionālo fizioterapiju.
Metodes: Datu meklēšana veikta BMJjournals, PubMed, Sciencedirect un Researchgate datubāzēs pēc atslēgas vārdiem “Chronic low back pain” AND “cognitive functional therapy” AND “exercise therapy” AND “Oswestry Disability Index” OR “Roland- Morris disability questionnaire” AND “Numerical Rating scale”.
Rezultāti: Sistemātiskajā literatūras apskatā analizēti trīs pētījumi, kur tika pētīta kognitīvi funkcionālās terapijas efektivitāte muguras lejas daļas sāpju un funkcionālās nespējas mazināšanā salīdzinājumā ar konvencionālo fizioterapiju. Visos trijos iekļautajos pētījumos tika iegūti ticami rezultāti, ka kognitīvi funkcionālā terapija ir efektīva metode sāpju un funkcionālās nespējas samazināšanai, taču tā nav efektīvāka par konvencionālo fizioterapiju.
Secinājumi: Kognitīvi funkcionālā terapija ir jauna, vēl maz izpētīta metode, līdz ar to literatūrā nav pieejami daudz pētījumi, kas pētītu šīs metodes efektivitāti sāpju un funkcionālās nespējas mazināšanai. Kognitīvi funkcionālā terapija ir efektīvāka metode īstermiņa rezultātiem lai mazinātu sāpes un funkcionālo nespēju pacientiem ar hroniskām muguras lejasdaļas sāpēm, taču ilgtermiņā efektīvāka ir konvencionālā fizioterapija.

Lower back pain has been shown to be a major cause of functional disability in people of working age, placing an economic burden on both the individual and the world. Back pain has been shown to be resistant to change and there is still a lack of clear evidence that one particular treatment is better than another in treatment of low back pain. Objective: To compile and analyse the available literature whether cognitive functional therapy is a more effective method in reducing chronic low back pain and functional disability than conventional physiotherapy. Methods: The literature search was performed in BMJjournals, PubMed, Sciencedirect and Researchgate databases. Keywords used were “Chronic low back pain” AND “cognitive functional therapy” AND “exercise therapy” AND “Oswestry Disability Index” OR “Roland-Morris disability questionnaire” AND “Numerical Rating scale”. Results: Three studies investigating the efficacy of cognitive-functional therapy in reducing lower back pain and functional disability compared to conventional physiotherapy were included in the systematic review. All three studies included showed reliable results showing that cognitive functional therapy is an effective method in reducing pain and functional disability, but it is not more effective than conventional physiotherapy. Conclusions: Cognitive functional therapy is a new method, so there are limited amount of studies available in the literature which would study the effectiveness of this method in reducing pain and functional disability. Cognitive functional therapy is a more effective method for short term outcomes to reduce pain and functional disability in patients with chronic low back pain, but conventional physiotherapy is more effective in the long term outcomes.
