Holecistektomija mūsdienās ir bieži veicama ķirurģiska operācija. To var realizēt ar atvērtu pieeju vai laparoskopiski. Laparoskopiska holecistektomija ir minimali invazīva, taču pēcoperācijas sāpes var būs arī ļoti intensīvas un var ietekmēt pacientu atlabšanas procesu. Viedokļi par vēlamāko atsāpināšanas metodi vēl joprojām atšķiras. Perifērie bloki var tikt izmantoti analgēzijai pie laparoskopiskām holecistektomijām. Šī pētījuma mērķis bija apkopot pieejamo literatūru par perifēro bloku kā multimodālās analgāzeijas komponentu pielietojumu pie laparoskopiskām holecistektomijām, analizējot to analgētisko efektu, pacientu apmierinātību un hospitalizācijas laiku. Literatūras avoti tika meklēti izmantojot "PubMed" datu bāzi 2024. g. oktobrī, izmantojot izvēlētos atslēgas vārdus, iekļaušanas un izslēgšanas kritērijus. Pēc literatūras atlases, 24 randomizēti, kontrolēti klīniskie pētījumi tika izvirzīti tālākai analīzei. Erector spinae plane (ESPB), quadratus lumborum (QLB), paravertebral (PVB), transversus abdominis plane (TAPB) and rectus sheath (RSB) bloki tika analizēti. Augstāk minēto bloku efekts analizēts balstoties uz sāpēm pēc operācijas, opioīdu /"rescue" medikamentu lietošanas, hospitalizācijas laiku, pacientu aktivizāciju un pacientu apmierinātību. Secināts, ka ESPB un TAPB var būt efektīvi un droši analgēzijas veidi pie laparoskopiskām holecistektomijām, tāpēc var būt ieteicami. QLB un PVB var būt efektīvs analgēzijas efekts, taču jāņem vērā bloku augstos riskus. RSB efektivitāte nav pietiekami skaidra un, iespējams, nepieciešams vairāk pētījumu par šo bloku.
Cholecystectomy is a common surgical procedure in modern times. It could be done either by an open surgery or laparoscopically. Laparoscopic cholecystectomy is minimally invasive, but postoperative pain can be severe and can affect patients' overall recovery. Opinions about analgesic method of choice still differ. Peripheral block could be used to provide analgesia in laparoscopic cholecystectomies. The aim of this study was to review accessible literature about peripheral blocks as a component of multimodal analgesic approach in laparoscopic cholecystectomies, analyzing its' effect on pain, patient satisfaction and time of hospitalization. Literature was searched using "PubMed" data base in October 2024 using chosen key words, inclusion and exclusion criteria. After reviewing found literature, 24 randomized, controlled clinical trials were included. Erector spinae plane, quadratus lumborum, paravertebral, transversus abdominis plane and rectus sheath blocks were analyzed. After reviewing and analyzing chosen clinical trials, effectiveness of said peripheral blocks were described based on analgesic effect, opioid /"rescue" medication use, patient activation, hospital stay and patient satisfaction. It was concluded that ESPB and TAPB may be an effective and safe analgesic method in laparoscopic cholecystectomy cases, therefore may be recommended. QLB and PVB could provide sufficient analgesia, but come with high risks. RSB may be researched more to provide more insight on its' effects.
