Darba nosaukums: “Rīšanas traucējumi pacientiem ar Parkinsona slimību”.
Pētāmā problēma: Parkinsona slimība ir lēni progresējoša un hroniska neirodeģeneratīva saslimšana (Cacabelos R., 2017). Vairāk kā 80% pacienti ar Parkinsona slimību saskaras ar rīšanas traucējumiem jeb disfāgiju, kuras sekas ir malnutrīcija un aspirācijas pneimonija (Suttrup, l., Warnecke, T. 2016). Svarīgi ir savlaicīgi konstatēt rīšanas traucējumus, lai piemērotu atbilstošu terapiju. Ar anketas palīdzību pacients, ārstējošais ārsts un audiologopēds spēs novērtēt pacienta rīšanas traucējumu simptomus un to izteiktību dažādās Parkinsona slimības stadijās.
Pētījuma mērķis: Izzināt rīšanas traucējumu klīniskās pazīmes pacientiem ar 1. - 4. stadijas Parkinsona slimību.
Uzdevumi: Izpētīt, kurā slimības stadijā indivīdiem ar Parkinsona slimību attīstās rīšanas traucējumi un izpētīt rīšanas traucējumu simptomu izteiktību un to daudzumu atkarībā no Parkinsona slimības smaguma stadijas.
Pētījuma metodes: Anonīma anketa “Rīšanas traucējumi pacientiem ar Parkinsona slimību” un Hoēna & Jāra (Hoehn&Yahr) skalas izvērtēšana. Anketa tulkota arī krievu valodā pie sertificēta tulka.
Pētnieciskais jautājums: Vai rīšanas traucējumu simptomi, kuri saistīti ar uztura uzņemšanu pacientiem ar Parkinsona slimību, korelē ar Parkinsona slimības smaguma pakāpi?
Pētījuma dalībnieki: 30 Rīgas Austrumu klīniskās universitātes slimnīcas, Neiroloģijas un Neiroķirurģijas klīnikas ambulatorie un stacionārie pacienti.
Pētījuma rezultāti: Biežākie pētījuma dalībnieku atzīmētie rīšanas traucējumu simptomi bija: sausas mutes sajūta (50%); aizrīšanās epizodes ar šķidru pārtiku (43,33%); siekalošanās ārpus maltītēm (33,33%); atklepošanās pēc kumosa norīšanas (33,33%).
Pētījuma secinājumi: Rīšanas traucējumu simptomi pacientiem ar Parkinsona slimību var izpausties jau agrīnās slimības stadijās. Pacientiem ar izteiktāku Parkinsona slimības stadiju, tika novērots lielāks rīšanas traucējumu simptomu daudzums. Rīšanas traucējumu simptomu pieauguma attīstība statistiski ticami korelē ar Parkinsona slimības stadiju.

More than 80% of patients with Parkinson’s disease experience swallowing difficulties, the consequences of which include malnutrition and aspiration pneumonia (Suttrup, l., Warnecke, T. 2016). Early detection of swallowing disorders is important for appropriate treatment. With the anonymous questionnaire, the patient’s treating doctor and speech therapists will be able to advise and plan therapy. Aim of the researh: to determine the clinical symptoms of swallowing difficulties for patients at stage 1 to 4 of Parkinson’s disease. Tasks: To determine the stage at which people with Parkinson’s disease develop swallowing difficulties and to research the severity of symptoms depending on the severity of Parkinson’s disease. Methods: Anonymous questionnaire “Swallowing problems in patient with Parkinson’s disease” and Hoehn & Yahr scale evaluation. The questionnaire is translated to Russian by a certified translator. Research question: Do swallowing problems associated with dietary intake in patients with Parkinson’s disease correlate with the severity of Parkinson’s disease? Research participants: 30 outpatients and inpatients of the Riga Eastern Clinical University Hospital, Clinic of Neurology and Neurosurgery. Results: The most common dysphagia symptoms were the following: dry mouth during the day (50%), choking on liquid food (43,33%), salivation not caused by food (33.3%), coughing after swallowing (33,33%). Conclusions: Symptoms of swallowing disorders in patients with Parkinson’s disease may appear in the early stages of the disease. Patients with more severe Parkinson’s disease had a higher incidence of swallowing symptoms. The development of an increase in symptoms of swallowing disorders is statistically significantly correlated with the stage of Parkinson’s disease.
