Darba nosaukums: Pleca atbalsta somas lietošanas efektivitāte, salīdzinot ar citām metodēm glenohumerālās locītavas subluksācijas aizkavēšanai un pleca locītavas sāpju mazināšanai personām pēc cerebrāla insulta: sistemātisks literatūras pārskats.
Darba mērķis: Noskaidrot vai pleca atbalsta somas lietošana ir tikpat efektīva kā citas metodes glenohumerālās subluksācijas aizkavēšanā un sāpju mazināšanā pleca locītavā līdz 6 mēnešiem pēc cerebrāla insulta.
Pētījuma metodes: Pētījumu meklēšanai tika izmantotas elektroniskās datu bāzes – PubMed, The Cohrane library, Clinical Key, Web of Science. Pētījumā tika iekļauta viena publikācija, kuras izdota 2017. gadā, pārējās publikācijas neatbilda atlases kritērijiem kā bezmaksas pieejas pilns teksts, izdošanas gads no 2010 līdz 2020, pētījuma tēma nebija ar insultu saistīta subluksācija un/vai sāpes pleca locītavā vai pētījuma dizains – randomizēti kontrolēti klīniskie pētījumi.
Rezultāti: Kopumā tika atrasti 322 pētījumi, pēc dublikātu atlases caur End Note tiešsaistes versiju palika 234 pētījumi. Iepazīstoties ar virsrakstiem un anotāciju, tika atmesti 204 pētījumi, jo tajos pētāmā grupa nebija insulta pacienti, netika apskatītas pleca locītavas komplikācijas. Pēc iepazīšanās ar pilnu tekstu, tika atmesti 29 pētījumi, jo tajos pētāmā grupa nebija insulta pacienti, netika salīdzinātas iejaukšanās metodes, tie nebija randomizēti kontrolēti pētījumi, nebija pieejams pētījuma pilns teksts vai pētījums vēl nebija beidzies. Iekļautais pētījums liecina, ka pleca atbalsta soma ir vienlīdz efektīva subluksācijas aizkavēšanai salīdzinot ar rokas pozicionēšanu uz riteņkrēsla galdiņa kombinācijā ar pleca atbalsta somu.
Secinājumi: Veikta pieejamo pētījumu par tēmu apkopošana; atrasts viens pētījums, kura vērtējums pēc PEDro skalas ir 9/11 un tas vērtējams kā izcilas kvalitātes. Pētījums rāda, ka pleca atbalsta somas ir tikpat efektīvas, kā rokas pozicionēšana uz riteņkrēsla galdiņa kombinācijā ar pleca atbalsta somu, lai aizkavētu pleca locītavas subluksāciju pacientiem pēc cerebrāla insulta subakūtā fāzē. Pleca atbalsta somas kombinācijā ar rokas pozicionēšanu uz riteņkrēsla galdiņa iespējams ir efektīva metode, lai mazinātu sāpes pleca locītavā pēc cerebrāla insulta subakūtā fāzē. Nepieciešama pētniecība par pleca atbalsta somas lietošanu gan pleca subluksācijas aizkavēšanā, gan sāpju mazināšanā pacientiem pēc cerebrāla insulta.
Objective: To determine whether shoulder slings are equally effective treatment for shoulder subluxation and shoulder pain compared to other interventions for patients after stroke. Methods: To compose systematic review a systematic search for publications will be held within databases - PubMed, The Cohrane library, Clinical Key, Web of Science. One publication was included in the review, others did not meet the inclusion criteria such as available free full text, published between 2010 and 2020, articles about stroke associated shoulder subluxation and/or pain and randomized controlled trial. Results: Total of 322 articles were identified before duplicate exclusion. After excluding duplicates 234 publications were left. After reading the titles and abstracts 30 publications were left to read the full text. After reading the full text only one publication met the inclusion criteria and was included in the review. Publication shows that using hemi sling is equally effective to prevent shoulder subluxation compared to wheelchair lap tray and triangular sling for patients after stroke. Conclusions: Compilation of available publications was done; one publication met the inclusion criteria and was rated by PEDro scale scoring 9/11 which is interpreted as excellent quality. Study shows that using hemi sling is equally effective to prevent shoulder subluxation compared to wheelchair lap tray and triangular sling for patients after stroke. Lap tray combined with triangular sling may be more effective in treatment of shoulder pain for subacute stroke patients. More research has to be done about shoulder sling treatment for shoulder subluxation and shoulder pain following stroke.
