Item type: Item , Backstroke Teaching Methods in Healthy Children with Residual Primitive Reflexes(2021) Bogdanoviča, Irina; Lāriņš, ViestursThere are studies suggesting that primitive reflexes can remain active in healthy children. Formal backstroke teaching is a part of many aquatic curriculum program. Even though the links with a problem during swimming learning with retained primitive reflexes were mentioned, there are no studies about effect of different backstroke teaching methods in healthy children with residual primitive reflexes. Aim of the study is to define the effect of teaching methods for backstroke in healthy children with residual primitive reflexes. Subjects: 6- 7 years old healthy children with residual primitive reflexes. Methods: A part practice backstroke teaching method for the first group (12 children) has been focused on teaching of separate components (leg kicks, arm pulls, and breathing) before fitting them together, while another 12 participants in second group learnt backstroke separate components of arm-to-leg integration before combining them together inside of cycle. After 12 weeks of practice backstroke proficiency and heart rate during maximal continuous backstroke swimming were analyzed. The obtained results show significant differences between the two groups. The results of the second group are linked with better backstroke proficiency, significantly longer swimming distance and lower heart rate during swimming.Item type: Item , Effects of Vestibular Exercises on Postural Balance for Children(2016) Kaupužs, Aivars; Lāriņš, Viesturs; Rižakova, LoritaThe balance is one of the most important skills of the postural control and ability to move. The data are gained during the research that was supported by National Research Program „Innovative solutions in social rehabilitation in Latvian schools in the context of inclusive education”. The balance sway index (SI)was tested by digital platform Bio Sway. The research group included 8 boys and 7 girls in the 12-14yearsage range with vestibular function insufficiency. The aim of the study was to examine the immediate effect of vestibular exercises and to evaluate the degree of postural balance parameter changes after vestibular exercises complex applications. Vestibular exercise with head rotation 20 seconds with 3 repetitions caused statistically significant (p-0.03) increasing of SI. The statistically significant (p-0.025) improvement of SI was set after vestibular rehabilitation exercises complex application during 6weeks (10-15 minutes a day).Item type: Item , Coherent Analysis of the EEG to Study the Correlation of Neural Connections of the Cerebral Cortex with the Cognitive Functions of 6–8 Y.O. Children Having Difficulties in Learning and Behavioral Problems(2023) Pudovskis, Konstantins; Lāriņš, Viesturs; Bondarenko, Andrejs; Tolmača, Nelli; Vandāns, JānisCurrent research is done in groups of children within the norm and from those having learning difficulties from preschool (6–7) and primary school ages (7–8). A comparative analysis of cognitive brain functions during orientation in a maze has been performed using coherent EEG analysis with respect to age-related features of the formation of neural connections in the brain cortex. Identified differences in the quantitative and spatial organization of neural connections in the cerebral cortex in both hemispheres of the brain. The dynamics of neural connections in children with learning difficulties indicate a pronounced age-related functional immaturity of the cortical regions of the brain, as well as disruptions in the regulatory brain structures – an imbalance between inhibitory and activating influences. The data from the coherent EEG analysis correlated with a high level of motor activity (often chaotic) in the maze, fragmented perception, unstable attention, low working memory capacity, and weak optimization of skills in mastering new spaces. Participants included 39 ballet dancers and 70 adolescents from ordinary school. Body composition was measured using a multi-frequency 8-polar bioelectrical impedance leg-to-hand analyser (X-Scan Plus II, Korea). Dietary intakes were assessed using a three-day estimated food record. Nutritional intake was calculated using the Nutri Survey software. Ballet dancers were slightly shorter, lighter, with less fat and fat-free mass compared to girls from ordinary school. 51.3% (95% CI 35.59 to 66.97) of ballet dancers and 4% (95% CI; 0.27 to 11.15) of ordinary school girls had a body fat level of 12% or less. The recommended amount of 35-45 kcal energy to kg fat-free mass for aesthetic sports was not reached by 42.1% (95% CI 27.61 to 50.65%) of ballet dancers. No statistically significant difference was found in percent body fat between ballet dancers who consumed energy less than the recommended amount compared to those who ate normally, but fatfree mass (p < 0.05) was lower in those who consumed 35-45 kcal energy to kg fat-free mass or less compared to those who ate more. Competitive composition in rhythmic gymnastics should not only meeting the requirements of the physical abilities, but being technically perfect and expressively performed by a gymnast as well. Describing the expressiveness of rhythmic gymnast’s performance, the term “special artistry” is used. Special artistry in rhythmic gymnastics means artistry of compositional content, artistry of rhythmic gymnast, shown by athlete during performance of competitional composition and gymnast’s artistic abilities, influencing aesthetical effect of composition performance. The evaluation of judges at competitions often has a subjective character that is why the actual task of research is to make the expert assessment more objective. Particular investigation in general focuses on possible solutions to make alternative judging. Thus special artistry generally is viewed in 4 components – technical, aesthetical, compositional and musical. This paper is focused only on compositional and musical components of special artistry, headlining creation, build and content of composition, its execution and compositional and musical skills of the gymnast. The aim of the research is to define and to classify competitional composition determining components criteria and exponents of special artistry in rhythmic gymnastics. To develop this research there was used the meta-analysis of scientific literature, using researches of the top rhythmic gymnastics, art and music specialists from one data base, with further creation of thematic typology of elements. In total there were screened 961 sources of which 107 qualified and 25 were chosen for further research. Investigating chosen data sources by the appropriated search terms, as a result 15 criteria and 104 exponents in compositional component, and 5 criteria and 4 exponents in musical component of special artistry in rhythmic gymnastics were determined and classified according to the selected structure of special artistry components in rhythmic gymnastics.Item type: Item , Case Study of Primitive Reflexes Impact on Swimming Skill Acquisition By Healthy Children(2020) Bogdanoviča, Irina; Lāriņš, ViestursPrimitive reflexes is an automatic muscle reactions which are only present in the first few months, but can remain active in healthy children, thus, impacting on motor proficiency. There is little evidence how primitive reflexes influence on swimming skills acquisition by healthy children. The aim of the study is to examine how primitive reflexes influence swimming skills acquisition by healthy children. Subjects: two 7-year-old girls. Methods: Checklists by Oh et al., (2011) were used to evaluate swimming skill proficiency at swimming lesson 8 and 16. The results were represented as a percentage of the possible maximal score. The testing of the asymmetrical tonic neck reflex, the symmetrical tonic neck reflex, and the tonic labyrinthine reflex was done using Blythe’s, 5-point rating scale. The analysis was carried out using descriptive method. The first girl showed two reflexes at level 1, while the second girl demonstrated all tested reflexes at level 1, 2 and 3. The girl with less total score of reflexes (8%) achieved a better level of swimming proficiency (from 73% to 77%) than the girl with higher (25%) severity of these reflex (from 21% to 33%.). Exergames have shown to elicit improvements in prefrontal cortex activity, balance, and postural control of seniors, all of which are associated with fall risk, but it is unknown whether virtual reality (VR) exergames, played using a three-dimensional headset can enhance the effects of cognitive and physiological functioning. Objective: Evaluation of the effects of a co-produced VR exergame “Falling diamonds” on physical performance, trunk stability and cognition, three attributes linked to falls risk in seniors. Methods: A total of 44 physically active participants aged 60–85 years were randomized to either the immersive VR exergame (n = 14), non-immersive exergame (n = 15), or control (n = 15). Static balance, leg strength, and gait speed were measured by the Short Physical Performance Battery, trunk stability was assessed using the Prone test and cognition was evaluated by the RehaCom screening soft-ware at baseline and follow-up at 9 weeks. Results: The VR exergame group experienced greater improvements in the cognition measures of selective attention control and speed (p = .009, p = .033) more than the exergame group (p = .010) and control (p = .049, p = .004). Conclusions: The evaluation and delivery methods of VR exergame Falling diamonds are feasible, and trial measures, procedures, and intervention are deemed acceptable by participants. Nevertheless, cognition and functional performance has been identifiedwith fall risks as well. The impact of functional performance elements and/or cognition on core stability, to complement the intervention programs for seniors in the future, has not been fully studied. This study aimed to assess the effects of leg strength, static balance, walking speed and cognition on the core stability scores. In a cross-sectional design study a sample of 62 participants were eligible for the study. Elderly aged 60 or older living in Social care centers or autonomous were recruited. Independent variables included static balance, walking speed, leg strength measured by Short Physical Performance Battery, cognition (alertness, divided attention, selective attention, and working memory) evaluated with the Rehacom screening software and age. Dependent variable core stability (the ability to maintain appropriate contraction of muscle transversus abdominis) was assessed performing Prone test withChattanooga Stabilizer Pressure Biofeedback. A hierarchical multiple regression was used for statistical analysis. Static balance and cognition presented greater effects significantly explaining 18% and 13% of the variability of core stability, whereas no significant effects of walking speed and leg strength were found on core stability. Exercise programs where static balance and cognition tasks are implemented to complement core stability performance mightbe worth investigating and compare with other intervention methods to reduce the risk of falling in the elderly population.Item type: Item , Mid-term outcome of distal osteotomy and first tarsometatarsal joint fusion for hallux valgus.(European Foot and Ankle Society Congress (EFAS) 2024 - Brussels, Belgium, 2024) Rumba, Evita; Stūrmane, Ērika; Egle, Alma; Jakušonoka, Ruta; Pavāre, ZaneObjectives: Many surgical methods have been described for correction of hallux valgus (HV), in general dictated by the severity of the deformity. The aim of the study was to evaluate mid-term outcome of patients with distal chevron osteotomy and Lapidus arthrodesis. Methods: A retrospective study of patients operated in the hospital from January 2017 to December 2018, was conducted. Two groups were identified: distal chevron osteotomy (group A) and Lapidus arthrodesis (group B). Pre-operative and mid-term postoperative radiological examination results and patients' satisfaction were analyzed. Results: Mid-term outcome was evaluated in 49 patients (48 female, 1 male), mean age 55.8 (31-77) years, mean 63 (45-81) months after operation. Eighteen patients (24 feet) were in the group A, 31 patient (35 feet) in group B. In the group A pre-operative mean hallux valgus angle (HVA) was 32.40, intermetatarsal angle (IMA) 11.50, distal metatarsal articular angle (DMAA) 29.50; postoperative mean HVA was 19.80, IMA 90, DMAA 160 (p<0.05). Mean grade of the medial sesamoid position pre-operatively was 2, postoperatively-1. In the Lapidus arthrodesis group pre-operative mean HVA was 38.10, IMA 150, DMAA 34.10; postoperative mean HVA was 160, IMA 8.60, DMAA 14.10 (p<0.05). Mean grade of the medial sesamoid position pre-operatively was 2, postoperatively-2. No significant difference among mean postoperative HVA, IMA, DMAA in group A and group B was identified (p˃0.05). In group A 14 patients (77.8%) were satisfied, 4 patients (22.2%) were unsatisfied; in group B 23 patients (74.2%) were satisfied, 8 patients were unsatisfied (25.8 %) with outcome. Two patients had superficial postoperative infection, one – hypertrophic scar, one – metatarsalgia, one stiffness. Discussion/conclusions: Both HV patients' groups demonstrated improvement of HVA, IMA, DMAA postoperatively. Two thirds of patients were satisfied with outcome in both groups. Patients are suggested to be informed about possible complications and risks after surgery.Item type: Item , Outcome for Patients with Acetabular Fractures Treated Using Only Modified Stoppa Approach or in Combination with Iliac Wing Approach: 10-Year Experience(Acta Chirurgiae Orthopaedicae et Traumatologiae Cechoslovaca, 2021-06) Vikmanis, Andris; Jakušonoka, Ruta; Jumtiņš, Andris; Pavāre, ZanePURPOSE OF THE STUDY This retrospective study presents our experience in treatment of polytrauma patients with acetabular fractures treated with only modified Stoppa approach or in combination with iliac wing approach. The purpose of this study was to evaluate the outcome of polytrauma patients with acetabular fractures operated with only modified Stoppa approach or in combination with iliac wing approach. MATERIAL AND METHODS Polytrauma patients who suffered from acetabular fractures treated in our hospital during 2008-2018, operated by using only modified Stoppa or in combination with iliac wing approach, were included in the study. The patients were evaluated using Majeed score. Cases with hip replacement due to post-traumatic osteoarthritis (PTOA) were summarized. The study group was compared with control group, which was operated through ilioinguinal approach. RESULTS We examined 42 patients (12 women and 30 men; mean age 44 years). Anatomical or satisfactory fracture healing was achieved in 92% of the acetabular fractures. In the study group Majeed score was significantly better (p<0.05). Seven patients from the study group had hip replacement due to PTOA with following results - 5 patients (13%) with anatomical or satisfactory reduction and 2 patients (50%) with displacement more than 3 mm of all reductions of acetabular fractures. DISCUSSION In polytrauma patients, the risk of the development of severe PTOA is increased because of high-energy trauma impact and a satisfactory reduction of acetabulum does not always protect against severe PTOA with following necessity for total hip replacement. CONCLUSIONS Our experience with using the modified Stoppa approach and its combination with iliac wing approach shows that these techniques provide better fracture reduction, gain mostly good radiological and functional results, may decrease the risk of development of severe symptomatic hip PTOA and hip replacement in polytrauma patients with both column acetabular fractures. Socks with integrated textile pressure sensors are a promising tool for non-obtrusive plantar pressure acquisition and monitoring. Research question: Can the DAid Pressure Socks System be employed for the measurement of plantar pressure and temporal parameters during walking? Method: Plantar pressure measurement was acquired simultaneously by the DAid Pressure Socks System (DPSS) and Pedar® insole measurement system for a normal walking gait of 17 volunteers. The measurement from both systems was processed and gait temporal parameters were calculated. The calculated temporal parameters included stride time, step time, stance time, swing time, single support time, double support time, and cadence. The Intraclass Correlation Coefficient (ICC) was calculated to evaluate the agreement between the plantar pressure measurement and temporal parameters obtained from both systems. Results: For plantar pressure measurement, excellent agreement (ICC > 0.90) was achieved in approximately 23 % cases, while in roughly 37 %, 27 %, and 13 % cases the agreement was good (ICC > 0.75), moderate (ICC > 0.50), and poor (ICC < 0.50), respectively. The mean relative differences (as a percentage) between the temporal parameter values calculated from the measurement of both systems were 0.80 % (stride time), 3.06 % (step time), 3.15 % (stance time), 5.08 % (swing time / opposite single support time), 14.47 % (double support time), and 0.78 % (cadence). Significance: The ICC result calculated for the plantar pressure measurement indicated a reasonable agreement between both systems. Additionally, the relatively low mean difference was observed for 5 out of 6 calculated temporal gait parameters. Consequently, the presented DPSS could be considered as a substitute for the commercial gait analysis tools for applications that do not require great accuracy of the absolute plantar pressure measurement.Item type: Item , LONG-TERM FUNCTIONAL OUTCOME AND QUALITY OF LIFE IN PATIENT AFTER SEVERE POLYTRAUMA(European Journal of Trauma and Emergency Surgery, 2017-05) Korņilova-Fiļušina, Vera; Vikmanis, Andris; Pavāre, Zane; Nulle, AndaCase History: A female of 30 years old had a severe polytrauma with multiple pelvic fractures, bilateral fractures of crural bones, traumatic injury of a.poplitea dxt. In spite of several efforts to perform vascular reconstruction of the right leg, the gangrene of the right foot continued to progress. This resulted in the amputation of the right leg at the middle third level of the femur. The pelvis was fixed with alocking plate at posterior access and an acetabular component with cannulated screw at anterior access. Clinical Findings: The patient’s physical state was evaluated according to the Amputee Mobility Predictor (AMP) (1). It was performed at 2 years post-polytrauma and resulted with a K3 functional level. Investigation/Results: Electromyography of the left lower extremity presented a n. tibialis sin. deep axonal injury above left knee joint. Diagnosis: Right side transfemoral amputation, secondary to traumatic etiology, n. tibialis sin. deep axonal injury above left kneejoint. Therapy and Progression: Majeed score (2) at 2 years post-polytrauma 88 points (excellent). The mobility according with modified Lower Extremity Functional Scale (LEFS) questionnaire (3) was 79% of the maximum possible. Comments: This case demonstrantes the significance of long-term follow-up with necessary out-patient treatment and rehabilitation of patients following severe polytrauma. Results indicate follow-up care improves the quality of life in patients. Modified LEFS could be used to evaluate the mobility of transfemoral amputees. However, the validity of these modifications must be determined by administering such modified questionnaire to a larger group of patients following transfemoral amputation with consequent statistical analysis of the received results. Early detection of excessive pronation and supination in running and walking gait are important for injury prevention not only for professional athletes but also for the general population. Unfortunately, the typical equipment necessary for detection of these conditions is relatively expensive and therefore is not widely used by non-professionals. Moreover, most of the typical equipment is either designed for operation under laboratory conditions only or is too bulky and consequently affects the performance of the wearer during the performed activity. This study proposes a method for pronation and supination detection by using smart textile socks with integrated pressure sensors and a specific sensor measurement processing algorithm. Each sock has five sensors, one on the heel, two in the middle and two in the front sole area. These sensors are knit from a conductive thread and have a resistivity inversely proportional to the pressure, which allows monitoring of the feet pressure at different parts during a physical activity. A special pressure vector is calculated from the sensor values to characterize the step. The value of this pressure vector give information about the step and allows detection of excessive supination and pronation in the gait. The preliminary tests demonstrated a significant difference between the pressure vector of a normal gait and gait performed with pronation and supination, thus allowing detection of these gait conditions.Item type: Item , Characteristics and Outcomes of Ankle Injuries in Winter and Summer Seasons(Proceedings of the Latvian Academy of Sciences, Section B: Natural, Exact, and Applied Sciences, 2020-10) Jakušonoka, Ruta; Arcimovičs, Toms; Vinčela, Gunita; Jumtiņš, Andris; Čerņavska, Ilze; Pavāre, ZaneThe diagnostics and treatment of ankle trauma remain challenging as they enable the patients' return to work and ability to perform daily activities. The aim of the study was to evaluate characteristics and outcomes of ankle injuries, focusing on the trauma mechanisms in winter and summer seasons. A retrospective study was conducted of 182 patients with ankle injuries, admitted to the Hospital of Traumatology and Orthopaedics in 2014, 2015, and 2016 from the months December to February, and from June to August. Patients with supination-external rotation (SER) type ankle injuries were included in the functional outcome evaluation. Our results suggested that most of the injuries were SER type, stage IV in both seasons, and that more than 2 3 of the SER type ankle injuries that needed surgery were in the winter season. In patients with tibiofibular syndesmosis (TFS) rupture the functional results were worse than in those without TFS rupture, but in winter and summer seasons they seemed to be without statistical difference.Item type: Item , Application of smart sock system for testing of shoe cushioning properties(IFMBE Proceedings, IFMBE volume 68/3, 2019) Oks, Alexander; Kataševs, Aleksejs; Eizentāls, Pēteris; Pavāre, Zane; Balčūna, DārtaAppropriate choice of shoes with required cushioning characteristics is rather an urgent problem for people from very different groups, such as sportsmen, elderly people, people with foot disorders and locomotion problems. Present research is devoted to further development of wireless DAids™ Pressure Sock System and its application for shoe cushioning estimation. In particular, a new version of pressure sensors with improved sensitivity and working range is designed and tested. Based on above-mentioned developments, the possibility of shoe cushioning testing using DAid™ Pressure Sock System was studied. For this purpose, gait records of several test subjects who used sets of shoes with different cushioning properties, as well as bare walking, were made. Data analysis showed that the developed system gives the possibility to recognize different shoe cushioning. Several approaches to data processing to increase the sensitivity of such recognition are discussed. The comparison showed the potential ability of the developed system to test wirelessly shoe cushioning in real outdoor conditions. The aim of our study was to evaluate the gait of patients with lower limb injuries in the medium-term after polytrauma. Three-dimensional instrumental gait analysis was performed in 26 polytrauma patients (16 women and 10 men; mean age 38.6 years), 14 to 41 months after the trauma. Spatio-temporal parameters, motions in pelvis and lower extremities joints in sagittal plane and vertical load ground reaction force were analysed. Gait parameters in polytrauma patients were compared with a healthy control group. Polytrauma patients in the injured side had decreased step length, cadence, hip extension, maximum knee flexion, vertical load ground reaction force, and increased stance time and pelvic anterior tilt; in the uninjured side they had decreased step length, cadence, maximum knee flexion, vertical load ground reaction force and increased stance time (p < 0.05). The use of the three-dimensional instrumental gait analysis in the evaluation of polytrauma patients with lower limb injuries consequences makes it possible to identify the gait disorders not only in the injured, but also in the uninjured side.Item type: Item , Mid-term outcome of patients with pelvic and acetabular fractures following internal fixation through a modified Stoppa approach(Acta Orthopaedica Belgica, 2013-12) Vikmanis, Andris; Jakušonoka, Ruta; Jumtiņš, Andris; Pavāre, ZaneThis retrospective study presents the mid-term outcome of polytrauma patients with pelvic ring and acetabular fractures operated using a modified Stoppa approach. We examined 32 patients (13 women and 19 men ; mean age 44 years) 24 to 40 months after the trauma. We performed a functional assessment of these patients using Majeed’s score. Mean Majeed score was 86.9 ± 8.22 with a range from 70 to 100. Excellent clinical results were noted in 59% of cases (in 83% of pelvic fractures and 40% of acetabular fractures) and good clinical results in 41% (in 17% of pelvic ring fractures and 60% of acetabular fractures). Fracture healing with anatomical reduction was achieved in all pelvic ring fractures. The most important task of the coach is to find the most efficient way how to prepare an athlete so that he would achieve the highest capacity and would be able to implement it. Application of cranial electrical stimulation in sport is not fully explored. The aim of the study was to determine the influence of cranial electrical stimulation on the strength parameter indicators of fitness athletes who use weightlifting tools. In the study participated 10 men (representatives of fitness sport, of Latvian Academy of Sport Education, who use weightlifting tools in the training process). The age of these athletes was 21±3 years. The average weight of the athletes was 76±3.2kg and the average height was 156±15.5cm. An exercise set "lifting a weight bar to the chest" was developed. A 10 minute session of cranial electrical stimulation was applied. During the test a weightlifting bar of the company "Eleiko" was used and FiTRO Dyne Premium cable was attached on one end of the bar which was connected to a computer system and registered the data obtained during the control exercise performance. During the experiment the results obtained during two tries of control exercise performance were compared, which are the difference between the average strength parameters and the difference between the maximum strength parameters before the application of cranial electrical stimulation (hereinafter – CES), performing the first three repetitions, comparing them to the last three repetitions performed after the application of the stimulation. After cranial electrical stimulation the maximum strength parameters improved by 533.23±11.09N (α<0.05). Fitness sport representatives and coaches, in order to improve the level of training of their student, must pay attention to the application of CES in the training process with weightlifting tools.
