Abstract: Streptokoku tonsilofaringīta precīziem diagnostikas indikatoriem ir būtiska nozīme tālākajā šīs slimības ārstēšanas gaitā, ņemot vērā gan izmaksu efektivitāti, gan iespējamo komplikāciju novēršanu. Šis pētnieciskais materiāls ir tematisks literatūras apskats, kurā tika veikta zinātniskās literatūras datu analīze. Darbā tika izmantoti dati no pētnieciskiem žurnāliem, interneta avotiem. Pētījuma datu analīzē tiek salīdzināti dažādi laboratorijas instrumenti, piemēram tiek salīdzināta A grupas streptokoku tonsilofaringīta ātrā antigēna noteikšanas testa un orofaringeālās kultūras testa efektivitāte, lai izvērtētu vienas vai otras šīs metodes priekšrocības. Lai veiktu precīzu streptokoku tonsilofaringīta diagnostiku un ārstēšanu, tiek izmantoti Centor kritēriji, kas ir ļoti būtiski ģimenes ārstam lai izvērtētu tālāko terapijas taktiku, antibakteriālās terapijas uzsākšanu. Viens no šī pētījuma būtiskākajiem uzdevumiem ir rast atbildi uz jautājumu kurā brīdi un kā uzsākt farmakoloģisko terapiju šīs grupas pacientiem. Pētījumā tiek analizēta dažādu antibakteriālo līdzekļu efektivitāte. Ir zināms - jo vairāk antibiotikas lieto, jo lielāks ir antibiotiku rezistences veidošanās risks. Darbā analizētā tēma primārajā veselības aprūpē ir ļoti būtiska, jo tonsilofaringītu var izraisīt dažādi vīrusi un baktērijas, bet slimības pazīmes un simptomi lielākoties ir nespecifiski. Lasītājs šajā literatūras apskatā noskaidros, ka ātrais antigēnu noteikšanas tests ir visrentablākā diagnostikas metode streptokoku tonsilofaringīta noteikšanai pieaugušajiem. Tomēr, lai apstiprinātu streptokoku tonsilofaringītu, rīkles kultūras tests ir specifiskāka un labākā metode. Centor kritērijus vislabāk izmantot kā papildinājumu citiem laboratorijas testiem. Un, visbeidzot, Penicillin joprojām ir pirmā izvēles metode, lai ārstētu streptokoku tonsilofaringītu. Protams tiek izmantoti arī citi antibakteriālie līdzekļi, kas varētu būt tikpat efektīvi kā penicilīns.
Indicators of diagnosis and therapy of streptococcal tonsillopharyngitis will address the main indicators, leading to an appropriate and cost-effective streptococcal tonsillopharyngitis diagnosis and therapy in general physician’s practice. This research paper is a thematic literature review where relevant facts will be collected from different trusted journal publishers and other relevant sources. The facts from different research papers and the personal opinion of the author of this literature review, the opinion and conclusions of different journal publishers will be well delimitated in order for the reader to follow this work. A comparison of the efficacy of different laboratory tools like rapid antigen detection test for group A streptococcus tonsillopharyngitis and oropharyngeal culture will be compared to each other in order to determine which method is the most appropriate to use. Centor criteria will be evaluated to determine the most specific symptoms for a proper assessment of streptococcus tonsillopharyngitis diagnosis and treatment. As well to determine which symptoms will let the general practitioner know when to start antibiotic treatment. Carriers of group A tonsillopharyngitis microorganisms are a problematic group that the general practitioner will encounter with the question when to or if to start antibiotic treatment. Asymptomatic carriers could be able to pass the streptococcal infection to other patients, especially in pediatric population and if in that case those children would need to have a pharmacological treatment therapy. When it comes to the pharmacological treatment, many questions arises and one of the purpose of this research paper is to facilitate answers to it. The efficacy of different antibiotic will be also evaluated. It is known that the more antibiotic use, the higher the risk of developing antibiotic resistance. Therefore comparing different antibiotic agents will reveal the least and most resistant antibiotics normally used in the general practitioners office. This research paper is highly important in the primary care because tonsillopharyngitis infection may be caused by a variety of virus, bacteria, among other microorganisms and the sign and symptoms are most of the time unspecific. The reader will find in this literature review that Rapid antigen detection test is the most cost-effective diagnostical methods in adult for the detection of streptococcal tonsillopharyngitis. However a throat culture test is more specific and the best method to confirm streptococcal tonsillopharyngitis infection. Centor criteria is best used as a complement to other laboratory tests. And lastly, Penicillin is still the first method of choice to treat streptococcal tonsillopharyngitis but there are other antibiotics that could be as effective as penicillin.
