Abstract: Dentīna paaugstināta jutība ir izplatīts zobu stāvoklis pieaugušo pacientu vidū, parasti periodonta problēmu dēļ: smaganu recesijas vai zobu nodiluma dēļ. Pacients sūdzas par akūtām sāpēm termiskas, osmotiskas vai mehāniskas iedarbības dēļ. Šī pētījuma mērķis ir salīdzināt dentīna paaugstinātas jutības ārstēšanas iespējas. Izmantojot elektroniskās datubāzes, par šo tēmu tika veikta elektroniska publikāciju meklēšana no 1999. līdz 2021. gadam. Pētījumi ir parādījuši, ka vidēji no šīs slimības cieš 10-30% iedzīvotāju. Pacientu vidējais vecums ir 20-50 gadi, galvenokārt sievietes. Paaugstinātas jutības pret dentīnu etioloģija ir daudzfaktoru, kas saistīta ar dentīna iedarbību uz mutes vidi, lietojot emalju vai cementu. Ne visiem indivīdiem, kuri parāda dentīna iedarbību, ir paaugstināta jutība pret dentīnu. Tas var būt saistīts ar atšķirīgo dentīna morfoloģiju, ne visi kanāliņi ir patenti vai arī uztriepes slāņa veidošanās dēļ. Darbības mehānisms nav pilnībā izprotams, bet visizplatītākā ir M. Brannstrēma piedāvātā hidrodinamiskā teorija. Šī teorija ir balstīta uz hidrodinamiskā mehānisma hipotēzi: autors uzskatīja dentīna kanāliņu par kapilāru cauruli, kas satur šķidruma vai celulozes transudātu, kas ir viegli pārvietojama gaisa, augstas temperatūras, aukstuma, zondes gala ietekmē. Šī strauja, iekšēja vai ārēja šķidruma kustība kanāliņos izraisa dentīna spiediena izmaiņas. Tas galu galā palielina pulpas vai dentīna kanāliņu nervu galu aktivitāti. Lai veiksmīgi novērstu paaugstinātu jutību, ir jāpiemēro sarežģīta pieeja. Pirmkārt, tas ir nepieciešams, lai identificētu DH cēloni un tieši iedarbotos uz etioloģisko faktoru. Zobu paaugstinātas jutības ārstēšanu var veikt divos virzienos: nervu šķiedru desensibilizācija un dentīna kanāliņu obturācija. To var izdarīt gan izmantojot birojā, gan mājas aģentos.
Dentinal hypersensitivity is a widespread dental condition among adult patients, typically due to periodontal problems: gingival recession or tooth wear. The patient complains of acute pain due to thermal, osmotic, or mechanical action. This research aims to compare the treatment options for dentin hypersensitivity. An electronic search of publications from 1999 to 2021 was made using the electronic databases, concerning this topic. Studies have shown that, on average, 10-30% of the population suffers from this disease. The average age of patients is 20-50 years, primarily women. The etiology of dentin hypersensitivity is multifactorial, associated with dentin exposure to the oral environment by wear of enamel or cementum. Not all individuals who show dentin exposure experience dentin hypersensitivity. This might be due to different dentin morphology, not all tubules are patent or due to formation of smear layer. Mechanism of action is not fully understood , but the most widespread is the hydrodynamic theory proposed by M. Brannstrom. This theory is based on a hydrodynamic mechanism hypothesis: the author considered the dentinal tubule as a capillary tube containing fluid or pulp transudate, which is easily displaced under the influence of air, high temperature, cold, probe tip. This rapid, inward, or outward movement of fluid in the tubules leads to a change in dentin pressure. It ultimately increases the activity of the nerve endings in pulp or dentinal tubules. To successfully eliminate hypersensitivity, it is necessary to apply a complex approach. First of all, it is needed to identify the cause of the DH and act directly on the etiological factor. The treatment of hypersensitivity of teeth can be carried out in two directions: desensitization of nerve fibers and obturation of dentinal tubules. It can be done both using in office and in home agents.
