Abstract: Kopš 1954.-2010. gadam tāds perorālais antikoagulants jeb K vitamīna antagonists (KVA) kā varfarīns bija vienīgā pieejamā terapija ātriju fibrilācijas (ĀF), dziļo vēnu trombozes (DVT) un citu indikāciju ārstēšanai. Tomēr, kopš pasaules tirgū ienāca tādi tiešas darbības (tiešie) perorālie antikoagulanti (TOAK) kā dabigatrāns, rivaroksabāns, apiksabāns un edoksabāns, to lietošana ir radījusi pārsvaru, salīdzinājumā ar KVA. Tiešas darbības (tiešie) perorālie antikoagulanti ir ērti lietojami medikamenti, jo tiem nav nepieciešama regulāra pacienta monitorēšana un devu pielāgošana. Tomēr ir pacientu grupas, kurām to veikt ir nepieciešams. Tie ir gados vecāki pacienti, pacienti ar samazinātu nieru funkciju, kā arī tie, kuri pakļauti polifarmācijai. Pētnieciskā darba tēma ir "Tiešas iedarbības perorālo antikoagulantu terapijas uzraudzība ilgtermiņā - sastopamie riski un iespējamierisinājumi". Darba mērķis bija identificēt potenciālus riska faktorus pacientiem, kas lieto TOAK, izanalizēt tos un aktualizēt nepieciešamību veikt pastiprinātu uzraudzību pacientiem, kā arī ierosināt priekšlikumu pilnvērtīgākai pacientu uzraudzībai/novērošanai ikdienas praksē. Pētījumā tika noskaidrots, ka riska faktori ir saistīti ar gados vecākiem pacientiem, it īpaši sievietēm, kā arī ar nepieciešamību lietot vairākus medikamentus vienlaicīgi.
Since 1954-2010 oral anticoagulants or vitamin K antagonists (CVAs) such as warfarin were the only therapies available for the treatment of atrial fibrillation (AF), deep vein thrombosis (DVT) and other indications. However, since direct-acting (direct) oral anticoagulants such as dabigatran, rivaroxaban, apixaban and edoxaban have entered the world market, their use has led to an advantage over CVA. Direct-acting oral anticoagulants are easy-to-use medications as they do not require regular patient monitoring and dose adjustment. However, there are groups of patients who need to do this. These are patients at an advanced age, patients with impaired renal function and polypharmacy patients. The topic of the research is "Long-term monitoring of direct-acting oral anticoagulant therapy - risks and possible solutions". The aim of the work was to identify potential risk factors for patients using NOAC, to analyze them and to update the need for increased attention and examination of patients, as well as to propose a more comprehensive examination of patients in daily practice. The study found that risk factors are patients at an advanced age, especially women, as well as patients taking multiple medications at the same time.
