The Ethics of Pacemaker Reuse from the Perspective of Distributive Justice

Abstract

Kardiovaskulārās slimības ir viens no biežākajiem nāves iemesliem pasaulē, un elektrokardiostimulatori ir bieži lietotas terapeitiskas ierīces, kas ļauj glābt dzīvības un palielināt dzīvildzi daudziem pacientiem katru gadu. No ētiskās perspektīvas zinātniskā literatūra par elektrokardiostimulatoriem pārsvarā koncentrējas uz jautājumiem, kas skar dzīves beigas. Tomēr pastāv vesela virkne citi, ne mazāk svarīgi ētiski jautājumi, kas skar elektrokardiostimulatorus, kuriem līdz šim ir pievērsta niecīga uzmanība un kuri nav pietiekami apspriesti un izpētīti. Šī pētījuma mērķis ir noskaidrot, vai un kādos apstākļos elektrokardiostimulatoru atkārtota lietošana būtu ētiska. Balstoties literatūras interpretācijā par tēmu un teorētiskajās pieejās sadalošajam taisnīgumam (egalitārisms, taisnīgums kā godīgums un utilitārisms), šis pētījums tiecas izskatīt argumentus pastāvošajā literatūrā un pielietot minētās teorētiskās pieejas sadalošajam taisnīgumam, lai analizētu elektrokardiostimulatoru atkārtotas lietošanas gadījumu. Teoriju komplicētība, kā arī to atšķirības liek domāt, ka universālas atbildes nav, reizē uzrādot, ka šīs atšķirīgās pieejas paver iespēju refleksijai par nosacījumiem, kam jāizpildās, lai elektrokardiostimulatoru atkārtota lietošana tiktu uzskatīta par ētisku.

Cardio-vascular diseases are one of the most frequent causes of death in the world with pacemakers being a frequent therapeutical device allowing to save and improve life years for many patients each year. From an ethical point of view most of the literature about pacemakers has focused on the end-of-life issues. However, there are a number of other—and not less important—ethical issues around pacemakers which have received little to no attention and have not been sufficiently discussed and researched much until now. The aim of this research was to determine if and under what conditions pacemaker reuse could be considered ethical. By being based on the interpretation of various literature on the topic and on different theoretical approaches to distributive justice (egalitarianism, justice as fairness and utilitarianism), it tries to review the arguments in the existent literature and to apply those theoretical approaches to distributive justice to the case of pacemaker reuse. The complexity of the theories as well as their differences have made it apparent that no universal answer could be found for this question but that different thought processes might allow reflection about the conditions needed for the reuse of pacemakers to be ethical.
