Abstract
Sirds un asinsvadu slimības ir galvenais nāves cēlonis visā pasaulē, tāpēc šo pacientu zobārstniecības aprūpe ir īpaši svarīga. Šis literatūras apskats tika veikts, lai gūtu ieskatu esošajos pētījumos par sirds slimnieku zobārstniecības aprūpi, apskatot biežāk sastopamās sirds slimības, kā hipertensija, koronārā sirds slimība, sirds aritmijas, infekciozs endokardīts, kā arī aprūpes principus pacientiem, kuri saņem antikoagulantus un antiagregantus. Lai nodrošinātu pacientu drošu aprūpi, ir svarīgi, lai zobārsts izprastu slimības procesus, farmakoloģiskos līdzekļus, kas tiek izmantoti dažādu sirds un asinsvadu slimību ārstēšanai, izprastu iespējamo mijiedarbību starp pacienta medikamentiem un zobārsta kabinetā lietotajiem farmakoloģiskajiem līdzekļiem, kā arī iespējamos provocējošos faktorus. Būtiski ir izprast zobārstniecības procedūras laikā nepieciešamo pacienta datu monitorēšanu, kas ļauj identificēt nevēlamu kardiovaskulāru notikumu. Šīs zināšanas palīdzēs nodrošināt drošu vidi un kontrolēt faktorus, kas potenciāli var izraisīt vai pasliktināt sirds un asinsvadu slimības.
Cardiovascular diseases are the leading cause of death globally and these patients are therefore frequently encountered in the dental practice. This literature review was carried out to gain insight in the existing research regarding the dental care of a cardiac patient by zooming in on the most prevalent cardiac conditions that are commonly seen in the dental practice, including; hypertension, ischemic heart disease, cardiac arrhythmias, infective endocarditis and patients on anticoagulant- and platelet therapy. In order to provide safe dental care it is crucial that the dentist needs to understand the disease process, the pharmacologic agents that are used to treat a variety of cardiovascular diseases, understand possible interactions between patient’s medication and pharmacological agents used within the dental office, as well as the potential trigger factors and what to monitor during the dental procedure in order to identify the presence of an adverse cardiovascular event. This knowledge will help to provide a safe environment and to control factors that potentially can initiate or worsen the cardiovascular disease.
