Abstract
Darba nosaukums: Vēdera priekšējās sienas muskulatūras vingrinājumu efektivitāte diastasis recti abdominis mazināšanai pēcdzemdību periodā: sistemātisks literatūras pārskats.
Pamatojums: Diastasis recti abdominis ir bieži sastopama problēma grūtniecēm dzemdību un pēcdzemdību periodā. Ziņots, ka izplatība ir 70% grūtniecības pēdējā trimestrī, 60% 6 nedēļas pēc dzemdībām un 30% 12 mēnešus pēc dzemdībām. Ja netiek veiktas atbilstošas iejaukšanās var tikt kavēta vēdera un tā funkciju mehāniskā kontrole. Fizioterapija un konservatīva ārstēšana ir primārā izvēle DRA ārstēšanai, taču vēl aizvien nav vienota viedokļa par efektīvākajiem vēdera muskulatūras vingrinājumiem DRA mazināšanai.
Mērķis: Apkopot un izanalizēt zinātnisko literatūru par vēdera priekšējās sienas muskulatūras vingrinājumu efektivitāti diastasis recti abdominis mazināšanai pēcdzemdību periodā.
Metodes: Pētījumi tika meklēti datu bāzes Primo, PubMed, ScienceDirect, EBSCOhost, ProQuest un PEDro. Pētījumi tika meklēti pēc atslēgas vārdiem: “diastasis recti abdominis” OR “DRA” AND “exercises” OR “physiotherapy” AND “postpartum” OR “after birth”. Pētījumi tika izanalizēti, veikta rezultātu analīze un secinājumi.
Rezultāti: Tika atlasīti 5 randomizēti kontroles pētījumi, kuros kā terapijas metode bija vēdera muskulatūras vingrinājumi. Kopā piedalījās 157 dalībnieces. Visos pētījumos tika novērota statistiski nozīmīga DRA samazināšanās pēc vingrinājumu terapijas.
Secinājumi: Vēdera muskulatūras vingrinājumi kombinācijā ar vēdera jostu vai rumpja stabilitātes vingrinājumiem ir efektīvāki nekā vēdera muskulatūras vingrinājumu programma atsevišķi. Vēl aizvien nav vienota protokola diastasis recti abdominis ārstēšanai pēcdzemdību periodā, kas noteiktu vingrinājumu izvēli, terapijas ilgumu un biežumu.
Title: Efficacy of anterior abdominal wall musculature exercises for reducing diastasis recti abdominis in the postpartum period: a systematic review. Background: Diastasis recti abdominis is a common problem in pregnant women during labor and postpartum. Prevalence has been reported to be 70% in the last trimester of pregnancy, 60% at 6 weeks postpartum, and 30% at 12 months postpartum. If appropriate interventions are not performed, mechanical control of the abdomen and its functions may be hindered. Physiotherapy and conservative treatment are the primary choices for the treatment of DRA, but there is still no consensus on the most effective abdominal muscle exercises for reducing DRA. Objective: To summarize and analyze the scientific literature on the effectiveness of anterior abdominal wall musculature exercises for reducing diastasis recti abdominis in the postpartum. Methods: The databases Primo, PubMed, ScienceDirect, EBSCOhost, ProQuest and PEDro were searched for studies. Studies were searched using the keywords: “diastasis recti abdominis” OR “DRA” AND “exercises” OR “physiotherapy” AND “postpartum” OR “after birth”. The studies were analyzed, results were analyzed and conclusions were drawn. Results: Five randomized control trials were selected that used abdominal exercises as a treatment method. A total of 157 participants participated. All studies showed a statistically significant reduction in DRA after exercise therapy. Conclusions: Abdominal exercises combined with abdominal girdle or core stability exercises are more effective than an abdominal exercise program alone. There is still no united protocol for the treatment of diastasis recti abdominis in the postpartum, which would determine the choice of exercises, the duration and frequency of therapy.
