Abstract
Monoredze ir klīnisks stāvoklis vecuma presbiopijas ārstēšanai, nodrošinot pacientiem skaidru redzi bez brillēm. Šo klīnisko stāvokli var sasniegt ar dažādām metodēm, no kurām viena ir IOL ievietošana acī kataraktas operācijas laikā. Tomēr pastāv diskusijas par to, vai monoredze patiešām būtiski uzlabo pacientu spēju skaidri redzēt bez brillēm. Pastāv arī uzskats, ka astigmatisma pacientu sasniegtais uzlabojums monoredzes operācijas rezultātā varētu būt būtiski sliktāks kā pacientiem bez astigmatisma.
Darba mērķis un uzdevumi:
Novērtēt pacientu vispārējo apmierinātību ar monoredzes kataraktas operācijas rezultātu. Noskaidrot, vai šī operācija ir būtiski uzlabojusi pacientu spēju veikt dažādas vienkāršas ikdienas aktivitātes bez brillēm, balstoties uz viņu subjektīvajām sūdzībām. Izvērtēt, vai pacienti bez astigmatisma operācijas rezultātā ir sasnieguši būtiski labākus uzlabojumus kā pacienti ar astigmatismu.
Materiāli un metodes:
Pētījums tika izstrādāts starp 20 Paula Stradiņa Klīniskās universitātes slimnīcas Oftalmoloģijas nodaļas pacientiem ar monoredzi pēc kataraktas operācijas, no kuriem 10 bija ar astigmatismu, bet 10 bez astigmatisma. Tika izvērtēta pacientu spēja veikt 9 vienkāršas ikdienas aktivitātes bez brillēm pirms un pēc monoredzes kataraktas operācijas. Grūtības veikt katru konkrēto aktivitāti tika izvērtētas ar punktu sistēmu (1 – 4 punkti). Tika vērtēta arī pacientu kopējā apmierinātība ar operācijas rezultātu, izmantojot punktu sistēmu (1 – 5 punkti).
Related Samples Wilcoxon Signed Rank Test tika izmantots, lai analizētu, vai monoredzes kataraktas operācija ir būtiski uzlabojusi pacientu spēju veikt katru konkrēto aktivitāti bez brillēm katrā no pacientu grupām atsevišķi. Mann-Whitney tests tika izmantots, lai izvērtētu, vai pastāv būtiska atšķirība saniegto uzlabojumu kvalitātē starp abām pacientu grupām. P vērtība < 0,05 tika izvēlēta par statistiski ticamu. Iegūtie rezultāti tika analizēti, izmantojot datu apstrādes programmu SPSS statistics un Microsoft Excel.
Rezultāti:
Analizējot iegūtos datus, Related Samples Wilcoxon Signed Rank tests uzrādīja, ka monoredzes kataraktas operācija ir būtiski uzlabojusi spēju bez brillēm veikt visas no deviņām vērtētajām ikdienas aktivitātēm pacientiem bez astigmatisma. Savukārt pacientiem ar astigmatismu tā bija būtiski uzlabojusi spēju veikt septiņas no deviņām vērtētajām ikdienas aktivitātēm. Pacientu kopējā apmierinātība ar operācijas rezultātu bija 96% pacientiem bez astigmatisma un 86% pacientiem ar astigmatismu. Mann-Whitney tests uzrādīja, ka nav būtiskas atšķirības operācijas rezultātā sasniegto uzlabojumu kvalitātē starp pacientiem bez un pacientiem ar astigmatismu.
Secinājumi:
Monoredzes kataraktas operācija var būtiski uzlabot pacientu spēju bez brillēm veikt dažādas ikdienas aktivitātes gan pacientiem bez, gan pacientiem ar astigmatismu. Pacientu sasniegto redzes uzlabojumu kvalitāte operācijas rezultātā būtiski neatšķīrās starp abām pētījumā iekļautajām pacientu grupām.
Monovision is a condition to treat age related presbyopia that enables patient to see clearly wihout glasses. This condition can be achieved through various methods, one of these methods is through IOL implantation into patients eye during cataract surgery. However, it is still discussed whether monovision significantly improves patients ability to see. It is also argued that astigmatic people might benefit less from the surgery than non-astigmatic patients. Aim 1) To find out patients general satisfaction with monovision cataract surgery results 2) To evaluate whether this surgery has significantly improved patients ability to do simple everyday tasks without glasses based on their subjective complaints 3) To evaluate whether there is a significant difference in the quality of surgery results between non-astigmatic and astigmatic patients Materials and methods Study was carried out on 20 Pauls Stradiņš Clinical University hospital patients with monovision after cataract surgery, where 10 were with and 10 without astigmatism. The difficulty doing 9 everyday tasks before and after surgery was evaluated by point system. Patients general satisfaction with surgery results was analyzed. Related Samples Wilcoxon Signed Rank test was used to determine whether monovision cataract surgery has significantly improved patients ability to do these 9 tasks. Mann-Whitney test was used to determine whether there is a difference in quality of surgery results between both groups. Reliability value of p=0,05 was chosen. Data were analyzed using software SPSS statistics and Microsoft Excel.
