Endodontisku materiālu izraisīta zoba kroņa daļas pārkrāsošanās – cēloņi un ārstēšanas iespējas

Abstract
Mūsdienu sabiedrībā ir ļoti augsti estētikas standarti, tas ir attiecināms uz jebkuru nozari, tai skaitā arī zobārstniecību. Šobrīd pievilcīgs smaids ar veseliem zobiem ir viens no skaistas sejas elementiem, kas rada pirmo iespaidu par cilvēku. Arvien biežāk pacientu galvenā sūdzība vēršoties pie zobārsta ir zobu pārkrāsošanās, kā arī vēlme uzlabot savu zobu estētisko izskatu. Pārkrāsošanās ir ārējo vai iekšējo zoba struktūru krāsas maiņa, kas pacientam rada būtisku estētisku problēmu, it īpaši, ja tas ir saistīts ar priekšzobiem. Izpratne par dažādiem materiāliem, kas var izraisīt zoba cieto audu pārkrāsošanos, ir svarīga klīnicistiem, kas ļauj ārstēšanas laikā, izvēloties alternatīvus materiālus un metodes zobu un sakņu kanālu ārstēšanai, izvairīties no zoba cieto audu pārkrāsošanās. Mērķis. Šī zinātniski-pētnieciskā darba mērķis bija apskatīt dažādu endodontijā lietojamo materiālu ietekmi uz zobu krāsu un izanalizēt pārkrāsošanās iemeslus, apskatīt alternatīvus ārstēšanas materiālus, kas neveicina zoba pārkrāsošanās, un aprakstīt ārstēšanas iespējas. Metodes. Tika izmantoti 49 literatūras avoti – raksti no žurnāliem. Meklēšana tika veikta elektroniskajās PubMed, ClinicalKey, ScienceDirect un Wiley Library datubāzēs. Tika atlasīti raksti angļu valodā no 1985. līdz 2020. gadam. Atslēgas vārdi: endodontic tooth discoloration, endodontic tooth discolouration, resorcinol formaldihyde discoloration, TAP discoloration, MTA discoloration, endodontic sealers discoloration, endodontic materials discoloration, nonvital tooth bleaching, discolored teeth management. Pēc atslēgvārdiem tika atrasti 1688 raksti sākot ar 1964.gada izdevumu. No atrastiem rakstiem tika izslēgti raksti, kas nebija atbilstoši izvēlētai tēmai, kas atkārtojas un ar līdzīgu informāciju, raksti vecāki par 1985. gadu, ka arī raksti, kuriem nebija pieejama pilnā raksta versija. Literatūras pārskatā tika izmantoti review, systematic review, meta-analysis tipa raksti. Rezultāti. Veicot 49 literatūras avota apskatu, tika atrasta šī zinātniski-pētnieciskā darba tēmai atbilstošā informācija. Tika atlasīta informācija par 4 endodontijā lietojamo materiālu grupām – skalošanas līdzekļi, intrakanālie medikamenti un ārstnieciskās pastas, sīleri, pildīšanas materiāli, kuri var izraisīt zoba pārkrāsošanos. Šī darba ietvaros tika apskatīti gan mūsdienās pielietojamie materiāli, gan tie, kurus šobrīd praktiski nelieto. Visbiežāk izraisīta pārkrāsošanās ir pelēkajos toņos, tā veidojas vairāku materiālu ietekmē. Zoba dentīns paliek tumšāks noteiktu materiālu iedarbības rezultātā un šis izmaiņas visizteiktāk var redzēt zoba cervikalajā rajonā, kur zoba emalja ir ļoti plāna un pārkrāsojies dentīns spīd cauri. Nav noskaidrots kādas vienas pārkrāsoto zobu ārstēšanas metodes pārākums attiecībā uz pārējām, bet noteikti labākus estētiskus rezultātus iespējams sasniegt ar protezēšanas palīdzību, tomēr arī iekšējā balināšana bieži dod labus rezultātus.
Modern society has very high aesthetic standards, it can be applied to any industry, including dentistry. Today, an attractive smile with healthy teeth is one of the elements of a beautiful face that creates a first impression of a person. Increasingly, the main complaint of patients going to the dentist is tooth discoloration, as well as desire to improve aesthetic appearance of their teeth. Discoloration is a change in colour of the external or internal tooth structures, which causes a significant aesthetic problem for patient, especially when it is associated with the incisors. Understanding the different materials that can cause tooth discoloration is important for clinicians to avoid dental discoloration during treatment by choosing alternative materials and methods for treating teeth and root canals. The aim. The aim of this research was to review the effects of different endodontic materials on tooth discoloration and to analyse the causes of discoloration, to review alternative treatment materials that do not cause tooth discoloration, and to describe treatment options. Methods. 49 literature sources were used - articles from journals. The electronic databases of PubMed, ClinicalKey, ScienceDirect and Wiley Library were searched. Articles in English from 1985 to 2020 were selected. Keywords: endodontic tooth discoloration, endodontic tooth discolouration, resorcinol formaldihyde discoloration, TAP discoloration, MTA discoloration, endodontic sealers discoloration, endodontic materials discoloration, nonvital tooth bleaching, discolored teeth management. 1688 articles starting with 1964 were found by keywords. Articles that did not correspond to the chosen topic, that are repetitive and contain similar information, articles older than 1985, as well as articles for which the full version of the article was not available were excluded from the articles found. Review, systematic review, meta-analysis type articles were used in this literature review. Results. A review of 49 literature sources revealed information relevant to the topic of the research. Information was selected about 4 groups of materials used in endodontics - rinsing agents, intracanal medications and medicated pastes, sealers, filling materials that can cause tooth discoloration. Within the framework of this work, both the materials used today and those that are currently practically not used were considered. The most common discoloration is in shades of gray, it is formed by several materials. Tooth dentin darkens as a result of exposure to certain materials, and this change is expressed most in the cervical region of the tooth, where the tooth enamel is very thin and the discolored dentin shines through. The superiority of one method of discolored teeth treatment over others has not been established, but better aesthetic results can be achieved by prosthetics, however, internal bleaching often gives good results.
