Abstract
Ebšteina anomālijas gadījums 4 gadu vecam pacientam, kuram tika veikta konusa procedūra, lai novērstu malformāciju. Anestēzijas vadība tika izstrādāta, lai bērns varētu ātri izsekot, lai spontāna ventilācija uzlabotu labā kambara darbību. Transstorakālā echo pielāgotā šķidruma vadība šajā sarežģītajā gadījumā ļāva ātri un veiksmīgi sasniegt rezultātu. Hemodinamiskās sekas kopā ar Ebšteina anomālijas pirmsoperācijas, intraoperācijas un pēcoperācijas sekām var būt izaicinošas un neprognozējamas. Šis gadījums parādīja sarežģītu Ebšteina anomālijas gadījuma veiksmīgu anestēzijas un ķirurģisku vadību.
A case of Ebstein’s Anomaly in a 4-year-old patient that underwent a cone procedure to repair the malformation. The anaesthetic management was engineered to ultra-fast track the child so that spontaneous ventilation would improve the right ventricular function. The transthoracic echo tailored fluid management enabled a rapid and successful outcome in this complicated case. The hemodynamic consequences along with the pre-operative, intra-operative and post-operative implications of Ebstein’s anomaly can be challenging and unpredictable. This case demonstrated the successful anaesthetic and surgical management of repair of a complicated case of Ebstein’s anomaly.
