Lasīšanas ātruma analīze pacientiem ar glaukomu

Abstract
Ievads.
Glaukoma ir viens no galvenajiem akluma cēloņiem pasaulē pacientiem, kas vecāki par 60 gadiem. Lai noteiktu, kuram pacientam indicēta ārstēšana, ir sīkāk jāizprot, kad un kā tieši glaukoma rada ar redzi saistītu invaliditāti. Šobrīd nav pietiekami daudz pierādījumu par to, kā agrīnās glaukomas stadijās tiek ietekmēta pacienta centrālā redzi.
Pētījuma mērķis.
Mūsu pētījuma mērķis bija noteikt lasīšanas ātrumu glaukomas pacientiem atbilstoši glaukomas stadijai ( agrīnai vai vēlīnai ), lai izprastu, vai agrīnās glaukomas stadijās ir novērojama centrālās redzes pasliktināšanās.
Metodes un materiāli.
14 pacientiem ar diagnosticētu glaukomu tika lūgts lasīt skaļi 1 minūti monookulāri ( ar labāk redzošu aci pēc pacienta vārdiem ) vienāda garuma un izmēra teksts no žurnāla. Lasīšanas apstākļi visiem pacientiem bija vienādi ( lasīšanas attālums bija 40 cm un telpas apgaismojums bija ap 400 luksiem). Visi pacienti spēja lasīt latviešu valodā, kā arī visi pacienti lasīja bez brillēm. Laiks tika mērīts, izmantojot hronometru. Par normālais lasīšanas ātrumu tika uzskatīts, ja pacients lasīja > 500 zīmēm / minūtē. Pacienti tika sadalīti divās grupās - pirmajā grupā bija pacienti ar agrīni atklātu glaukomu, bet otrajā grupā - pacienti ar vēlīnu glaukomu. Glaukomas stadija tika noteikta, izmantojot redzes lauka testu. Dati tika analizēti, izmantojot Microsoft Exel un IMB SPSS 22.0 (Mann - Whitney tests ).
Rezultāti.
Kopumā tika analizēti 8 vīrieši un 6 sievietes. Vidējais pacientu vecums bija 69,85 [SD = 6,29] gadi. Agrīnā glaukomas grupā vidējais lasīšanas ātrums bija 598 zīmes / min, bet vēlīnas glaukomas grupā - 234 zīmes / min. Tika konstatēta statistiski nozīmīga atšķirība starp lasīšanas ātrumu un analizētajām grupām (p = 0,002), tātad pacienti ar vēlīnu glaukomu lasīja krietni sliktāk kā pacienti ar agrīnu glaukomu. Netika atrasta statistiski nozīmīga atšķirība starp lasīšanas ātrumu un vecumu vai dzimumu (p> 0,05).
Secinājumi.
Pacienti ar vēlīni glaukomu lasīja tekstu ievērojami lēnāk, salīdzinot ar pacientiem agrīna glaukomas stadijā. Mūsu pētījums norāda, ka lasīšanas ātruma mērīšana kopā ar redzes lauka testu varētu labāk palīdzēt analizēt glaukomas stadiju un centrālās redzes nelabvēlīgu ietekmi. Lasīšanas ātruma analīze kā diagnostikas metode varētu palīdzēt precīzāk noteikt ar redzi saistīto invaliditāti pacientiem.
Objectives. Glaucoma is one of the leading causes of blindness for people over the age of 60.Determining who should be treated requires that we understand when and how glaucoma produces disability. However,there is not enough evidence to tell us when glaucoma produces vision-related disability.The aim of our research was to measure reading speed in glaucoma patients according to the stage of glaucoma. Methods. 14 patients with glaucoma were given exactly the same length and size text to read aloud for 1 minute monocularly. Reading conditions were the same for all patients(reading distance was 40 cm and room lighting was between 400 and 600 lux).The time was measured using chronometer.Normal reading speed was considered >500signs/minute. Patients were divided in two groups – in the first group were patients with early detected glaucoma but in the second group – patients with advanced glaucoma.Stage of glaucoma was measured using Visual field test.Data was analysed using Word Exel and IMB SPSS 22.0, Mann- Whitney test. Results. 8 men and 6 women were analysed. Average age of patients was 69,85[SD=6,29] years. In early glaucoma group the mean reading speed was 598 signs/min but in the advanced glaucoma group–234 signs/min. There was found statistically significant difference between reading speed and analyzed groups(p=0,002).There was not found statistically significant difference between reading speed and age or gender(p>0,05). Conclusion. Patients with advanced glaucoma were associated with slower reading speed compared to patients with early glaucoma. Our research indicates that reading speed measurement together with Visual field test could help better analyse the stage of glaucoma and the central vision defect. This could also help to determine the vison-related disability in patients.
