Abstract
Šī zinātniskā tēze izskaidro atšķirību starp akūtu miokarda infarktu un takotsubo kardiomiopātiju, kas ir divas svarīgas slimības, kas izraisa sirds audu bojājumus. Akūts miokarda infarkts ir dzīvībai bīstams stāvoklis, kas izraisa pastāvīgu sirds išēmiju pastāvīgu muskuļu bojājumu dēļ, savukārt takotsubo kardiomipātija ir īslaicīga išēmiska slimība, kas ietekmē sirds kambarus, lai efektīvi sūknētu asinis. Takotsubo kardiomiopātija ir daudz retāka nekā akūts miokarda infarkts, tādēļ tā tiek nepareizi diagnosticēta. Visizplatītākā līdzība ir klīniskā izpausme, abiem parasti ir tādi simptomi kā sāpes krūtīs, aizdusa un nogurums. EKG abos gadījumos ir redzams ST pacēlums, tas nozīmē, ka EKG nevar izmantot kā diagnostikas instrumentu to diferencēšanai, drīzāk angiogramma būs pareiza diagnostikas metode, lai tos diferencētu, papildus troponīna līmenī tos var arī diferencēt, kur akūta miokarda infarkta gadījumā troponīna līmenis būs daudz augstāka nekā takotsubo kardiomiopātijā. Ehokardiogrammā parādīs tipisku apikālo balonēšanu takotsubo kardiomiopātijas gadījumā, kuras nav akūtas miokarda kardiomiopātijas gadījumā. Ārstēšana ir atšķirīga, akūta miokarda infarkta gadījumā tūlītēja perkutāna koronārā iejaukšanās ir nepieciešama mazāk nekā 120 minūtes, ārstēšanu seko ar ilgstošiem medikamentiem, savukārt takotsubo kardiomiopātijas gadījumā var būt spontāna atveseļošanās, taču, iespējams, būs nepieciešamas zāļu vadlīnijas. Akūtam miokarda miokarda infarktam ir lielāks komplikāciju risks, kam seko slikta prognoze nekā takotsubo kardiomiopātija.
This scientific thesis elucidate the difference between acute myocardial infarction and Takotsubo cardiomyopathy which is two important diseases which cause heart tissue damage. AMI is a life threatening condition that cause permanent ischemia of the heart due to permanent muscle damage while Takotsubo cardiomypathy is a temporary ischemic condition affecting the heart chambers to pump blood effectively. Takotsubo cardiomyopathy is much rarer than AMI making it being misdiagnosed. The most common similarity is the clinical presentation, both are commonly having symptoms such as chest pain, dyspnea and fatigue. On ECG both show ST elevation, means ECG cant be used as a diagnostic tool to differentiate them but rather angiogram is better, in additional troponin level can also differentiate them where in AMI the troponin level will be much higher than in Takotsubo cardiomyopathy. ECHO will show typical apical balloning in case of Takotsubo cardiomyopathy which is not present in case of acute myocardial cardiomyopathy. In case of AMI immediate percutaneous coronary intervention is needed less than 120 minutes, the treatment is followed with long term medications while in Takotsubo cardiomyopathy there can be spontaneous recovery but might require medications guidelines. Acute myocardial myocardial infarction have higher risk for complication and poor prognosis than Takotsubo.
