Pacientu ar obstruktīvu miega apnoju līdzestība pozitīva gaisa spiediena procedūras laikā

Abstract
Darba aktualitāte. Obstruktīva miega apnoja ir visizplatītākais elpošanas traucējums miegā, kura izplatība pasaulē skar 10% populācijas. Arī Latvijā šai saslimšanai ir tendence pieaugt. Tas ir izskaidrojams ar sabiedrības pieaugošo interesi par savu veselību meklējot dažādus risinājumus tās uzlabošanā. Gan medicīnas personāls, gan pieejamā informācija interneta vidē aicina vairāk pievērst uzmanību miega kvalitātei un aicina izvērtēt miega apnoju veicinošos riska faktorus. Līdz ar pieaugošo hronisko slimību skaitu, tiek meklēti iemesli un piedāvātas dažādas izmeklēšanas un ārstēšanas metodes. Obstruktīvas miega apnojas ārstēšanas “ zelta standarts” ir pozitīva gaisa spiediena terapija. Darba autore vēlējās noskaidrot, cik līdzestīgi ir pacienti šīs ārstēšanas uzsākšanā un kādi ir tie faktori, kas motivē OMA pacientus to lietot regulāri.
Pētījuma mērķis. Noskaidrot, vai pastāv sakarība starp dažādu slimības smaguma miega apnojas pacientu grupām un iekārtas lietojumu ietekmējošajiem faktoriem - sociāli ekonomiskā stāvokļa un subjektīvās labsajūtas.
Hipotēze. Miega apnojas pacientu iekārtas lietojums pēc līdzestības kritērija ir atkarīgs no slimības smaguma, sociāli ekonomiskā stāvokļa un subjektīvās labsajūtas.
Pētījums tika īstenots dr. Andas Nodievas ārsta praksē pneimonoloģijā kā kvantitatīvs, retrospektīvs kohortas pētījums, kurā piedalījās 45 pacienti ar diagnozi vidēji smaga vai smaga obstruktīva miega apnoja. Pacientu vidējais vecums bija 61.2 gadi (SD=9.0 gadi) (n=45) un 67% no visiem pacientiem bija vīrieši. Korekta līdzestības rādījuma iegūšanai tika vērtēti trīs mēnešu iekārtas lietojuma dati.
Secinājumi. Veicot datu analīzi noskaidrojās, ka 75% pacientu pastāvīga pozitīva gaisa spiediena iekārtas lietošana atbilst noteiktajam kritērijam ≥ 4 stundām naktī un ir uzskatāma par pietiekamu, lai pacienti ar obstruktīvu miega apnoju sasniegtu ievērojamus veselības uzlabojumus.
Topicallity of thesis. Obstructive sleep apnea is the most common sleep-disorder, affecting 10% of the world's population. In Latvia, too, this disease tends to increase. This can be explained by the growing public interest in their health in the search for various solutions to improve it. Both the medical staff and the information available on the Internet call for more attention to be paid to the quality of sleep and for the risk factors for sleep apnea to be assessed. With the growing number of chronic diseases, the causes are being sought and various methods of examination and treatment are being offered. The "gold standard" for the treatment of obstructive sleep apnea is positive air pressure therapy. The author wanted to find out how equal the patients are in starting this treatment and what are the factors that motivate OMA patients to use it regularly. Objective of the study. To find out whether there is a connection between the groups of patients with sleep apnea of different severity of the disease and the factors influencing the use of the device - socio-economic status and subjective well-being. Hypothesis. The use of sleep apnea patient equipment according to the criterion of equivalence depends on the severity of the disease, socio-economic status and subjective well-being. The study was carried out by dr. Anda Nodieva's doctor's practice in pneumonology as a quantitative, retrospective cohort study involving 45 patients diagnosed with moderate to severe obstructive sleep apnea. The mean age of the patients was 61.2 years (SD = 9.0 years) (n = 45) and 67% of all patients were male. Three months of equipment usage data were evaluated to obtain a correct equivalence reading. Conclusions. Analysis of the data showed that 75% of patients used continuous positive air pressure equipment for the ≥ 4 hours at night criterion and were considered sufficient to achieve significant health improvements in patients with obstructive sleep apnea.
