Abstract
Polifarmāciju definē kā vismaz piecu medikamentu lietošana ikdienā. Polifarmācija ir izplatīta vecāka gadagājuma pacientu vidū un līdz ar to demogrāfiskā novecošana padara polifarmāciju par aktuālu jautājumu visā pasaulē. Farmaceits kā viens no veselības aprūpes speciālistiem, veicot farmaceitisko aprūpi, spēj mazināt polifarmācijas riskus. Vairākas valstis mēģina ieviest farmaceitu mājas vizītes pie pacientiem. Latvijā pirms tam netika veikti pētījumi par farmaceitu mājas vīzītēm. Pētījuma mērķis bija noteikt polifarmācijas izplatību un zāļu lietošanas paradumus Latvijā, kā arī farmaceitu vēlmi un gatavību veikt mājas vizītes pie pacientiem. Iegūtie rezultāti apliecina, ka Latvijas farmaceiti nav gatavi veikt farmaceitisko aprūpi mājas apstākļos.
Annotation Providing pharmaceutical care to patients in home and polypharmacy risk reduction. Polypharmacy is defined as the use of five or more medications daily by a patient. Polypharmacy is common in older people and therefore demographic ageing makes polypharmacy a worldwide issue. Pharmacist as a member of a healthcare team can reduce risks of polypharmacy by providing pharmaceutical care. Many countries try to integrate pharmaceutical care into home care. There has not been done any research on pharmacist home visit intervention in Latvia yet. The aim of the study was to find out the prevalence of polypharmacy and habits of using medicines in Latvia, in addition to pharmacists' opinions on home visits. The results show that pharmacists in Latvia are not ready to provide home visits.
