Abstract
Palielinās potīšu lūzumu skaits, un jo īpaši vecākiem cilvēkiem to saslimstība pieaug. Šo lūzumu ārstēšana ir ļoti svarīga jo potītes svara nesošās funkcijas dēļ, slikts iznākums var pasliktināt dzīves kvalitāti. Šī pētījuma mērķis ir noskaidrot, kā tiek pieņemts lēmums par ārstēšanu, kādi ir faktori, kas ietekmē šo izvēli. Pirmkārt, es izpētu lūzumu faktorus, piemēram, klasifikāciju, traumas veidu un pavadošās traumas. Otrkārt, es pētu pacientu faktorus, kas, iespējams, varētu ietekmēt ārstēšanas izvēli, piemēram, blakusslimības, dzimumu un vecumu. Literatūras apskats tika veikts, izmantojot vairākas datu bāzes, piemēram, EBSCO, Elsevier, ScienceDirect, PubMed, Scopus un Foot and Ankle Journal. Pētījumi liecina, ka šie faktori ietekmē izvēli, taču daudzas no tēmām joprojām tiek pētītas. Daudziem lūzumu faktoriem nav stingru vadlīniju, un bieži vien lēmumu pieņem ārstējošā ķirurga zināšanas un kompetence. Pacientu faktori ietekmē operāciju risku, palielinot komplikāciju biežumu, taču joprojām šķiet, ka tās ir paredzamas un pārvaldāmas pēcoperācijas periodā, tādējādi, iespējams, neietekmējot ārstēšanas kursa izvēli.
Ankle fractures are increasing in number and especially in the elderly population the incidence is increasing. Treatment of these fractures is highly important due to the weight bearing function of the ankle, and a poor outcome could lead to decreased life quality. The research aim is to find out how the decision about treatment is made and what are the factors that affect this choice. Firstly, I research fracture factors such as classification, type of injury, and concomitant injuries. Secondly, I research patient factors that could possibly affect the choice of treatment, such as comorbidities, sex, and age. The literature review was conducted using several databases such as EBSCO, Elsevier, ScienceDirect, PubMed, Scopus and Foot and Ankle Journal. Studies show that these factors affect the choice, but many of the topics are still under investigation. Many fracture factors do not have a strict guideline and often the decision is made based on the knowledge and competence of the treating surgeon. Patient factors affect the risk of surgeries by increasing the rate of complications, but it still seems that these are predictable and manageable in the postoperative period, thus possibly not affecting the choice of treatment course.
