Nefrektomija kā urīnceļu infekcijas prevencijas metode pacientiem pēc nieru transplantācijas

Abstract
21. gadsimtā orgānu transplantācija kļūst par rutīnas ārstēšanas metodi, kas prasa ne tikai plašas zināšanas par donoru atlasi, ķirurģiskām metodēm un pēc transplantācijas imūnsupresīvās terapijas izvēli, bet arī pacientu tālāku vadīšanu un pēc transplantācijas slimību un infekcijas pārvaldīšanu. Lai gan nieres transplantācija, kā ārstēšanas metode pacientiem ar terminālu nieru mazspēju, ir kļuvusi par izplatītu operāciju, joprojām ir vairāki aspekti, kas prasa padziļinātu izpēti, kā, piemēram, vienotu UCI prevencijas ķiruģiskas ārstēšanas vadlīniju izstrādi. Šis pētījums tika īstenots ar mērķi noskaidrot, vai profilaktiska nefrektomija samazina urīnceļu infekcijas biežumu pacientiem pēc nieres transplantācijas.
Darba mērķis. Izpētīt, vai profilaktiski veikta nefrektomija pacientiem pēc nieres transplantācijas ar UCI samazina to biežumu.
Pētījuma materiāli un metodes. Retrospektīvs šķērsgriezuma pētījums. Pētījumā dalībnieki tika atlasīti no Paula Stradiņa klīniskās universitates slimnīcas nieru transplantācijas pacientiem, kuriem laika posmā no 2015. līdz 2020. gadam Latvijas Transplantācijas centrā tika veikta nefrektomija. Dati tika reģistrēti un uzglabāti Microsoft Excel programmā. Anonimizēta datu apstrāde tika īstenota, izmantojot statistikas programmu IBM SPSS Statistics 29. Iegūto datu analīzei tika izmantota aprakstošā statistika, Spīrmena rangu korelācija, Man-Whitney tests un neparametriskais Vilkoksona tests, lai pārbaudītu izvirzīto hipotēzi par urīnceļu biežuma samazināšanos pēc veiktās nefrektomijas. Vilkoksona tests tiek izmantots divām kopu salīdzināšanai, kuras neatbilst normālajam sadalījumam, lai noteiktu vai pastāv statistiski nozīmīga atšķirība starp urīnceļu biežumu pirms un pēc nefrektomijas.
Secinājumi. Balstoties uz iegūtajiem datiem statistiski ticami, ka profilaktiski veikta nefrektomija pacientiem ar nieres transplantātu samazina UCI biežumu (p=0,003).

In the 21st century organ transplantation is becoming a routine treatment method that requires not only extensive knowledge of donor selection, surgical techniques, and the choice of post-transplant immunosuppressive therapy, but also patient management and post-transplant disease and infection management. Although kidney transplantation has become a common operation as a treatment method for patients with end-stage renal disease, there are still several aspects that require in-depth research, for instance development of unified surgical treatment guidelines for the prevention of UCI. The purpose of this study was to determine whether prophylactic nephrectomy reduces the incidence of urinary tract infection in kidney transplant patients. Purpose of work. To investigate whether prophylactic nephrectomy in renal transplant patients with UCI reduces their incidence. Research materials and methods. A retrospective cross-sectional study. In the study, participants were selected from kidney transplant patients of the Pauls Stradinš Clinical University Hospital who underwent nephrectomy in the Latvian Transplantation Center between 2015 and 2020 year. Data were recorded and stored in Microsoft Excel. Anonymized data processing was carried out using the statistical program IBM SPSS Statistics 29. Descriptive statistics, Spearman's rank correlation, Mann-Whitney test and non-parametric Wilcoxon test were used to analyze the obtained data to test the proposed hypothesis of a decrease in urinary frequency after nephrectomy. Wilcoxon test is used to compare two groups that do not follow a normal distribution to determine whether there is a statistically significant difference between the frequency of urinary tract infections before and after nephrectomy. Conclusions. Based on the obtained data, it is statistically reliable that preventive nephrectomy in patients with a kidney transplant reduces the frequency of UCI (p=0.003).
