Abstract
Epilepsija ir bieži sastopama saslimšana ar būtisku negatīvu ietekmi uz dzīves kvalitāti, ja netiek pilnvērtīgi ārstēta. Būtiskai epilepsijas pacientu daļai attīstās farmakorezistenta epilepsija un īr ir nepieciešamas jaunas alternatīvas terapijas metodes, ņemot vērā to, ka bieži vien ķirurģiska terapija ir kontrindicēta.
Neiromodulācija ir plaši pētīta terapeitiska alternatīva.Neiromodulācija ietver invazīvu neirostimulāciju un neinvazīvas metodes, kas izraisa intrakraniālu strāvas plūsmu. Šajā pārskata rakstā tiks apskatītas divas neirostimulācijas metodes; transkraniāla magnētiska stimulācija un transkraniāla līdzstrāvas stimulācija. Abas iepriekšminētās terapijas metodes pašlaik tiek pētītas dažādu neiroloģisku un psihiatrisku patoloģiju terapijā , ieskaitot epilepsiju.
Šajā apskata pārskata rakstā tiks apskatīti TMS un tDCS pamatprincipi, kā arī tiks aplūkoti tiks pētījumu par un drošumu un efektivitāti cilvēku pētījumos
Epilepsy is a common disease with serious negative effect on quality of life if poorly managed. A significant portion of epilepsy patients develop drug resistant epilepsy and new alternative treatment methods are needed as surgery is often also contraindicated in these patients. Neuromodulation with electricity is one of the options under research. Methods include invasive neurostimulation, and non-invasive methods transmitting electric current through the scalp. In this review the focus will be in two non-invasive neurostimulation types: transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS). Both methods are currently being studied as therapy options for a variety of psychiatric and neurological pathologies, including epilepsies. In this review, we will not only go through the basic concepts of TMS and tDCS but also review relevant human studies to evaluate the possible efficacy and safety of these methods on epileptic patients.
