    Antiplatelet Resistance in Patients with Atherosclerosis

    Variable platelet response to aspirin and clopidogrel is a well-known phenomenon in patients with coronary artery disease and ischemic cerebral stroke. The objective of the present study was to evaluate the frequency and possible risk factors of antiplatelet resistance in patients with cerebrovascular and cardiovascular diseases. The VerifyNow system was used to evaluate adenosine-5-diphosphate and platelet P2YI2 receptor function in patients with cerebrovascular and cardiovascular disease, who received dual antiplatelet therapy. Aspirin resistance was defined as aspirin reaction units (ARU) >= 550. Clopidogrel resistance was defined as Platelet Reaction Units (PRU) > 230. In the group of cerebrovascular diseases there were 13.2% (n = 27) patients with aspirin and 24.5% (n = 50) with clopidogrel resistance. However, in the cardiovascular group there were 20% (n = 9) aspirin and 11.1% (n = 5) clopidogrel resistant patients. In the cerebrovascular group, aspirin resistant patients had a lower triglyceride level (p = 0.001, r = 0.26) than aspirin sensitive patients. Clopidogrel resistant patients had a significantly higher level of glycated haemoglobin (HbA1C) (p = 0.016, r = 023), triglycerides (p = 0.033, r = 0.16) and lower level of high-density lipoproteins (p = 0.027, r = 0.16) than clopidogrel sensitive patients. In the cardiovascular group, patients who were resistant to aspirin had a significantly higher high-density lipoprotein level (p = 0.038, r = 0.31). No other factors differed significantly between the aspirin or clopidogrel resistant and sensitive patients in the cardiovascular group. Aspirin resistance was more common in patients with cardiovascular disease, and clopidogrel resistance in patients with cerebrovascular disease, although the difference was not significant. Our findings indicate that diabetes mellitus and an elevated level of lipoproteins could be risk factors for aspirin or clopidogrel resistance in patients with cerebrovascular diseases. Further studies should be conducted using larger patient cohorts with balanced groups of patients to investigate clinical aspects of antiplatelet resistance.

    Building the Road to Green Entrepreneurial Orientation in Higher Education and Research : Sharing Experience and Looking Ahead

    The Green Entrepreneurial approach is an important trend in strengthening education and research in Europe and in the world. The article examines a concept of green entrepreneurship and development of “skills ecosystems” as a dimension in university education and research. This methodology serves as a tool for integrating a number of main goals focused on providing high quality vocational skills to young people and contributing to regional deve lopment, innovation, smart specialisation and social inclusion. The article discusses how educational institutions must respond to the challenges in providing a high-quality research-training environment. The authors show that the establishment of dual education and related research at university level is an evident strategy for higher education institutions as this will give students an opportunity to acquire solid knowledge about subject matters and contribute to build a stimulating research environment. This contribu tion aims to offer insight into the implications in relation to the need to expand and adapt the content and the approach of Green Entrepreneurship and innovation in dual educational programmes and research in the EU and Latvia. The development of analytical skills and specialist knowledge promoted by dual education is a particular asset in areas, where in-depth knowledge of the green entrepreneurial orientation approach is key to research excellence.

    Clinical and Epidemiological Characteristics of Migraine in Latvia

    The aim of this study was to explore clinical-epidemiological characteristics of migraine in Latvia and the disability of migraine patients, to determine the optimal options for care of these patients. Patients selected from three headache centres were asked 40 questions by phone. Among 116 patients, the dominant age group was 25-34 (41.4%) and most patients were women (87.9%). 52.6% of patients were highly educated, and 53.4% had a job involving communication with people. In 56.9% of cases headaches started at the age of 15-24. About 25% migraine attacks lasted for 25-48 hours. The diagnosis "Migraine with aura" was confirmed in 50% of the patients. Headaches were quite frequent: 4-8 times a month or more in 22.4% of patients and 2-4 times a month in 29.3%. As medication, 50.9% used selective serotonin 5-HT (5-hydroxytryptamine) receptor agonists for migraine attacks treatment, 35.3%-nonsteroidal anti-inflammatory drugs (NSAIDs) and 30.2%-acetaminophen-containing medicines. The average quality of life estimated by migraine disability assessment questionnaire (MIDAS) was 13.0, which equates to moderate disability. According to our study clinical characteristics of migraine in Latvia do not differ significantly from that of other European countries. Most of the patients are educated, working women of childbearing age. Migraine diagnosis and treatment seems to be appropriate, but nevertheless, too many sufferers have frequent and persistent headache attacks that require further investigation.

    Ciešas ES un CA valstu attiecības var veicināt gan ES, gan Centrālāzijas valstu ekonomisko attīstību un nostiprināšanos. Viena no sadarbības prioritātēm ir ekonomiskās attīstības veicināšana, tirdzniecība reģionā un ES investīciju piesaiste reģionam. 2007. gadā ES apstiprināja pirmo stratēģiju ES attiecībām ar Centrālāziju, veicinot sadarbību reģionā. 2019. gadā tika atjaunota ES-Centrālāzijas stratēģija. Jauns elements ir sadarbība digitālajā ekonomikā, koncentrējoties uz piekļuves atvieglošanu pārvaldāmam, atvērtam un drošam platjoslas savienojumam un digitālajai infrastruktūrai, tostarp pieņemot nepieciešamās regulatīvās reformas. Tirgus atvēršana ārvalstu investoriem, uzņēmējdarbībai un investīcijām labvēlīga vide kalpotu kā līdzeklis ekonomikas diversifikācijai un ārvalstu tiešo investīciju un konkurētspējas palielināšanai. Darba mērķis ir izpētīt ES attīstības sadarbības stratēģiju un atbalsta programmu lomu ekonomiskās un uzņēmējdarbības vides konkurētspējas un ilgtspējas veicināšanā Centrālāzijas reģionā; izvērtēt ES un Centrālāzijas reģionālās partnerības tālākas virzības potenciālu, sniedzot ieteikumus efektīvai un sistemātiskai pieejai ES un Centrālāzijas institūcijām un uzņēmējiem tālākiem uzlabojumiem. Darba struktūru nosaka pētījuma mērķis, uzdevumi un tā loģika. Pirmajā nodaļā tiek veikta ekonomikas attīstības un izaugsmes koncepciju metateorētiskā analīze, izmantojot monogrāfiski aprakstošo, analītisko un grafisko metodi un holistisko pieeju ekonomiskās un uzņēmējdarbības vides metodoloģijai. Darbā sniegti secinājumi un ieteikumi ES-Centrālāzijas partnerības attīstības veicināšanai un Centrālāzijas valstu, kā palīdzības saņēmējvalstu, konkurētspējas stiprināšanai.     The European Union New Landscape: Impetus for Sustainable Economic Growth in Central Asia

    This article explores the problems of the EU in the challenging times for the world economy, Brexit, EU solidarity towards external policies and, in relation to countries in Central Asia. We examine the EU regional approach from two perspectives: the socio-economic situation as an integral component of the overall transformation process in Central Asia, and from the perspective of political economy of the regional cooperation perspectives of this region’s countries with the EU. This methodological approach serves as a tool of integrating several main goals related to the EU’s soft power that require the development and support of civil society and promotion of European values in Central Asia. This contribution aims to offer insight into the implementation of the EU external policies approach towards countries in Central Asia, taking into consideration implications of the new political and economic realities in the EU. Also, further economic development in the region of Central Asia as a precondition for the prosperity and security of countries in the region is discussed. The article concludes that the EU external policies have inevitably become more important due to the new economic and political powers emerging in Asia.

    Improving Digitalization Through Co-Creation: Case Of The Citadel Project

    This article examines problems related to digitalization and use of digital services provided by the government. The study is based on the research work and results of the CITADEL project, which has tested co-creation methods in Latvia considering requirements and interests of society and businesses in decision making processes of public administration. It appears that in many cases people aren’t willing to accept proposed services as they haven’t been involved in the processes of their creation. While enjoying benefits provided by information technologies, electronic services and artificial intellect people still need an exchange between human beings to make a good use of existing advanced solutions. This is especially important for electronic services widely used by a large part of the population and enterprises, which need to be easy, fast and friendly. The concept of co-creation or co-production has been initially developed in the 1970s emerging from theoretical and empirical analysis of urban service delivery. Nowadays the co-creation is becoming a priority of many governments in the world and scholars in public administration see the potential of ICTs in this exchange process. Governments around the world are promoting an increased use of ICTs to predict and understand the complexity of public services, as well as to improve the transparency and efficiency of government practices and facilitate democratic practices using e-government solutions. This paper aims at contributing to the concept of co-creation and the impact of digitalisation of public services. The article concludes that efficient decision-making for improving economic development, as well as social welfare at regional, national and local levels needs implementation of digitalised services applying new modern approaches, such as the co-creation as they inevitably become more important due to the increased digital competitiveness of countries.

    Promoting the Regional Competitiveness Through Clusters' Approach: Case of the Latvian Information Technology Cluster

    The purpose of this research is to contribute to the debate on regional competitiveness concept by evaluating genesis of the studies of concept development, and the impact assessment. This paper also presents the impact of clusters on regional competitiveness through the experience of the Latvian Information Technology (IT) cluster. The methods used in the research are literature analysis, correlation and statistical analysis. The main tasks include: (i) the usage of competitiveness concept to describe main factors of regional competitiveness, (ii) identification of the role of clusters for regional competitiveness by testing the linkage between regional competitiveness and state of cluster development, (iii) presenting the experience of the Latvian Information Technology (IT) cluster and the assessment of advantages enterprises operating in the cluster environment. The novelty of the research is testing the relationship between regional competitiveness and state of cluster development, as well as introducing the assessment of the CEOs of the Latvian IT cluster enterprises on advantages of cluster environment. In addition, synthesis of the cluster’s concept has led to findings on mandatory and desirable features of clusters. The authors conclude that regions with higher competitiveness have better developed clusters, while regions with higher state of cluster development are more competitive. The experience of Latvian IT enterprises in the cluster’s environment recognize that more can be achieved by working together than if acting alone. However, benefits of belonging to cluster aren’t always recognized and exploited.

    Role of EU Strategies and Support Programmes in Economic and Business Environment Enhancement in Central Asia. Doctoral Thesis

    Close EU-Central Asian relations can contribute to the economic development and strengthening of both the EU and Central Asian countries. One of the priorities of cooperation is the promotion of economic development, trade in the region and attraction of the EU investments to the region. The EU endorsed the first strategy for EU relations with Central Asia in 2007, promoting cooperation in the region. In 2019, the EU-Central Asia strategy was renewed. The novel element is cooperation in digital economy, focusing on facilitating access to manageable, open and secure broadband connectivity and digital infrastructure, including through the adoption of necessary regulatory reforms. Opening the market to foreign investors, business and investment-friendly environments would serve as a means of economic diversification and increasing foreign direct investment and competitiveness. The aim is to study the role of the EU development cooperation strategies and support programmes in enhancing the competitiveness and sustainability of the economic and business environment in the Central Asian region; to evaluate the potential for advancing the EU-Central Asia regional partnership, providing recommendations for an efficient and systematic approach for the EU and Central Asian institutions and business entities to achieve further improvements. The structure of the Thesis is determined by the aim, objectives, and logic of the research. The first Chapter covers a meta-theoretical analysis of economic development and growth concepts applying the monographically descriptive, analytical, and graphical method and a holistic approach to the methodology of the economic and business environment. The chapter specifies sustainable and competitive business environment factors from a viewpoint based on Sustainable Development goals and indicators. The second Chapter discusses results and indicators of transition to market economy, assesses the EU strategies and support programmes, their genesis and impact on sustainable economic and business development in Central Asian region. The third Chapter is devoted to the analysis of the sustainable growth and Central Asian economic and business environment based on the results of empirical research, mostly interviews with experts from the EU and Kazakhstan. The Thesis provides conclusions and recommendation related to the development of the EU-Central Asia partnership for strengthening competitiveness of Central Asian countries as beneficiaries. The main text of the Thesis is 181 pages. The list of References contains 323 sources. There are 20 Tables, 6 Boxes with essential information, 30 Figures, 3 Maps and 8 Annexes.
